Lot 12, Gardenia Street, Garden Royale Villas, Pasig City after having sworn to in
accordance with law, hereby depose and say:

That I am one and the same person as Sittie Hashema M. Macmac;

That my school records bears my name as Sittie Hashema M. Macmac instead

of Sittie Hashima M. Macmac;

That it was a plain clerical error in my school records;

That I am executing this affidavit to attest to the truth of the foregoing facts
and to show that SITTIE HASHIMA M. MACMAC and SITTIE HASHEMA M. MACMAC
are one and the same in my person, and to support my application for Japan Tourist
Visa.
