PROCEDURES :
2. Concerned DOLE Regional Office evaluates the application and schedules a validation/panel
interview.
3. DOLE Regional Office conducts final evaluation of the application and decides whether to
approve or disapprove it.
4. If approved, DOLE issues Certificate of Accreditation and an ID card after payment of the
corresponding fees.
REQUIREMENTS:
Applicant must have completed the Bureau prescribed 40-hour Basic Occupational Safety and
Health Training from DOLE accredited or recognized organizations.
Must have relevant experience in OSH -
- Three (3) years experience is required if applicant is duly licensed.
- Four (4) years experience, if applicant is a graduate of any 4 or 5 year college course without
any PRC license
- Ten (10) years experience if applicant is a college undergraduate.
Reminders:
All pages of documents submitted must be signed by the applicant and original of all documents
submitted must be presented to the authorized evaluator during applicant’s interview.
Accreditation shall be valid for three (3) years and shall be renewed upon compliance with
minimum requirements pursuant to D.O. 16, unless suspended, cancelled or revoked.
Accreditation shall be renewed within the prescriptive period of 30 calendar days prior to
expiration thereof.
