PETITION

COMES NOW, the undersigned petitioner unto this Honorable

Court, most respectfully states that:
1. Petitioner possesses the eligibility for commissioning as Notary
Public as shown by his following circumstances:

a. Citizenship: Filipino
b. Date of Birth: 12 July 1990
c. Residence: Lawaan Village, Barangay Balantang, Jaro,
Iloilo City
d. Contact No.: 09206018908

2. He has satisfactorily completed his law degree at Central

Philippine University on April 2018; passed the Bar Exams of 2018
and admitted to the practice of law on 14 June 2019 and with
office address at E. Lopez St., Jaro, Iloilo City, Philippines;

3. He is a member of the Philippine Bar with Roll of Attorney’s

Number 87000 and is applying for appointment of his notarial
commission for and within this court’s territorial jurisdiction;

4. He has paid his membership due on 10 June 2019 at Manila
City with IBP Membership due receipt Number 123456 hereto
attached as Annex “A”;

5. His professional Tax Receipt No. 0123456 was issued on 25 June

2019 at Iloilo City hereto attached as Annex “B” and made and
integral part thereof,

6. He is a member of the bar of good standing and of good moral

character with no pending administrative/ disbarment case as
evidenced by the Certification issued by the Office of the Bar
Confidant of the Supreme Court, Certification as to payment of
Required Membership Dues CY 2019 and Certification from the
Integrated Bar of the Philippines National and Local Chapter.
Copies of the said clearances are herewith attached as Annex
“C”, Annex “D”, Annex “E”, and Annex “F” respectively and
made an integral part of the petition;

7. He possesses all the qualifications and none of the

disqualifications to said office and has never been convicted
of any crime involving moral turpitude;

8. He has not applied nor does he intend to apply for a

commission in any other province or city in the Philippines;

9. That he has paid the corresponding application fee for his

Notarial Commission, a copy of the Official Receipt is hereto
attached as Annex “G”;

10. Also attached and marked as Annex “H” is another sheet

paper containing three specimens of his signature as well as his
passport size colored photographs with signature at the bottom
taken within 30 days of the petitioner with signature at the
bottom part;

11. A copy of Certificate of Membership in the Philippine Bar

issued by the Office of the Bar Confidant of the Supreme Court
is hereto attached as Annex “I”, and madean integral part
hereof.

 WHEREFORE, it is most respectfully prayed of this Honorable
Court that the Petitioner be appointed as as Notary Public for and
within this court’s territorial jurisdiction for the term commencing 01
January 2020 to 31 December 2022.

Other reliefs just and equitable are likewise prayed for.

3. The allegations herein are true and correct based on my

personal knowledge and authentic records;

4. To the best of knowledge, there are no other pending petitions

of the same nature before any court, tribunal, or quasi-judicial
agency; and

5. Should I thereafter learn of any similar case pending before any

court, tribunal, or quasi-judicial agency, I shall promptly notify
this court within five (5) days from discovery.
