A. PURPOSE AND BACKGROUND

The investigators of this study would like to ask for your participation. Before you decide to take part in
this study, it is important that you understand why the research is being done and what your participation
will involve. Please read the following information carefully and feel free to ask the researchers if there is
anything that is not clear or if you need more information.

The purpose of this study is to know how different ways of teaching affects the academic performance of
the students.
B. PROCEDURES
If you agree to participate in this research study, the following will occur. Answering from part 1: General
Weighted Average (GWA) this first semester S.Y. 19-20 to part 2, where you will put a check ( ) on the specific box
which corresponds your answer. The survey will take up to 5 minutes approximately.
C. RISKS
Participation in this test is entirely voluntary and there are no known or anticipated risks for participating.
If you happen to feel uncomfortable, embarrassed, and/or inconvenienced, you may decline to answer
any or all questions and you may withdraw your participation at any time if you choose.
D. CONFIDENTIALITY
All information you provide will be kept in utmost confidentiality and would be used for academic
purposes only. No individual identities will be used in any reports or publications resulting from the study.
E. BENEFITS OF PARTICIPATION
There will be no direct benefit to you, but your participation is likely to help the researchers find out more
about the variables involved in the topic.
