The MST (Mentor School Teacher) will use this form to formally observe the trainee’s performance and to give
formative feedback based on the selected teaching competencies. The use of letter grades is optional.

Mariam consistently keeps detailed formative and summative assessment to inform planning. Formative and
summative assessment strategies are evident in all lesson plans.

Mariam completes reflections which consider situations and reflects on relationships with students as part
of professional practice and uses the insights to improve these relationships.
