CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT is made and entered into by and between:

xxx, a sole proprietorship duly organized and existing under and by virtue of
the laws of the Republic of the Philippines, with principal office at Mactan
Newtown, Lapulapu City, hereinafter referred to as the “COMPANY”, and
represented in this act by its Chief Executive Officer, xxx, of legal age, Filipino,
single and a resident of Camputhaw, Cebu City; and

xxx, a sole proprietorship duly organized and existing under and by virtue of
the laws of the Republic of the Philippines, with principal office at Gun-ob,
Lapulapu City, hereinafter referred to as the “CONTRACTOR”, and represented in
this act by its Chief Executive Officer, xxx, of legal age, Filipino, married and a
resident of Gun-ob, Lapulapu City,

WITNESSETH:

1. That the CONRACTOR shall make available its services to the

COMPANY and agree to undertake the following:

a. To perform an interior fit-out of the COMPANY’s office located at

Mactan Newtown, Lapulapu City;

b. To renovate the said office according to the design and specifications

agreed by the parties;

c. To provide for the necessary labor and materials; and

d. To complete this project on or before August 5, 2019.

2. That for and in consideration of the services to be rendered, the

COMPANY holds, binds and obligates to pay the CONTRACTOR the
project cost in the amount of Php500,000.00.
 IN WITNESS WHEREOF, parties hereto have hereunto set their hands this

This instrument which refers to CONTRACT OF SERVICE is consisting of

two (2) pages including the page whereon this acknowledgement is written has been
signed by the parties and their witnesses on the left margin on each and every page
thereof and sealed with my notarial seal.
