1.1 All medical staff have a job description including
non-medical duties (as per client instruction) that
describes their role

3.0 SUPPORT FACILITIES AND AMBULANCE


3.1 There is adequate firefighting equipment in the
facility

that reachable for 24/7
3.3 First Aid Facilities have a PC with internet access

3.4 Ambulance are well maintained with appropriate


records of maintenance
3.5 Ambulance are clean inside and outside

3.6 There are seat belt for driver and passenger

3.7 There are copies of all driver's licenses

3.8 The driver of ambulance is trained in first aid

3.9 There is appropriate life support equipment for the

4.0 MEDICAL SERVICES

4.1 On questioning, medical staff can explain the


principal of informed consent
4.2 Staff are able to describe the procedure to be taken
if a patient withdraws their consent
4.3 All patient records are kept in a secure place

4.4 There is log book that contains the name, age,


symptoms, preliminary diagnosis and treatment
both on hard file and soft file for statistical purposes
4.5 The standard forms used for patient care are well
presented KOC, Petrofac, and CCC logos
4.6 Medical records show that S.O.A.P is the
methodology used for documentation
4.7 There is a handover procedure with rotational staff
that is documented
4.8 There are clinical practical guideline and
nursing/medic services

5.0 EMERGENCY SYSTEM


5.1 There is a MERP (Medical Emergency Response
Procedure) which are clearly displayed on the wall
5.2 There are written protocols and algorithms for
emergency management which are clearly displayed
on the wall
5.3 There is recorded evidence of a MERP exercise
within the last 6 months
5.4 Updated material safety data sheet (MSDS) for the
chemical used on site available in the medical
facility
5.5 All medical staff shall read and understand the
MSDS and form for signed after read the MSDS
provided as evidence

6.0 MEDICINE
6.1 Emergencies medicines included Anti-Allergy,
Analgesic medicine etc.is available
6.2 All medicine that added and removed from
inventory are recorded
6.3 The monthly utilization of stock is known and
documented

6.4 The temperature of the refrigerator is checked and


recorded twice daily
6.5 Room temperature of the area where pharmacy is
stored are maintained and recorded twice daily
6.6 All pharmacy items are stored on a first in first out
basis
6.7 All pharmacy and disposable items are in date

7.0 INFECTION CONTROL


7.1 Sharps containers available

7.2 Sharps containers are sealed and kept in a locked


area prior to being removed from the facility
7.3 All biomedical waste that removed from the facility
is documented
7.4 Bins for medical and general waste are segregated

7.5 All bins are foot operated with fitting lids

7.6 There are adequate hand washing facilities in the


medical facility
7.7 Step for correct hand washing procedure are clearly
posted on the wall

8.0 MEDICAL RECORD


8.1 Are Fitness Certificates for all employees available at
First Aid Facilities or accessible for medical
personnel on site?
8.2 Are Medical Records for all employees available at
