1. ENGINEERING DESIGN AND CONSTRCTION SUPERVISON OF THE PALMA – AFUNGI ROAD

1.1 QUALITY PLAN AND OBJECTIVES

To ensure construction procedures is compliance to specification and standard and to maintain consistently the
quality of work.

1.2 EXECUTIVE SUMMARY

1. Materials Laboratory is not complete:
a. Calibration certificates are not at site
b. Some of equipment are not complete
c. Borrow pit is not yet sample, due to machinery (excavator breakdown), this is very critical for contractor
production. E-mail was communicated to the contractor Quality Engineer dated 5/17/2018 8:53 AM/ our
ref: SWM-CCCM-LN-001
d. No official test result submitted for approval
e. Sampling road bed at ch. 0+200 for analysis

2. Production: no. 3 teams

a. Ongoing of cleaning and grubbing, approximately at km 10+300 complete, contractor have to submit RFI
for approval
b. Ongoing of roots picking (cleaning) of road bed preparation, approx. 500 meters complete from 0+00 to
0+500 of team 1, contractor have to submit RFI for approval
c. Ongoing of roots picking (cleaning) of road bed preparation, approx. 700 meters complete from 4+000 to
4+700 of team 2, contractor have to submit RFI for approval
d. Team 3 with Dozer cleaning road diversion for traffic for production machinery
e. Ongoing preparation of camp accommodation and site offices for workforce

SHE SUMMARY

Daily Toolbox Talks are presented @ 06H35 before start of any activities. All employees on site have attended Site Safety
Induction training both CCCM, Scott Wilson Moz and Geocontrole.

Workshop and equipment inspection checklists are being completed on a daily basis and inspected by CCCM HSE supervisor

HSE have been conducting random drug and alcohol testing on the site as part of the desire to see a proactive approach and
ensure the site is kept alcohol and drug free.

A few safety concerns have been observed/raised and discussed at toolbox. Operators and drivers are being monitored
regarding usage of seat belts and PPEs
