CERTIFICATE OF EMPLOYMENT

This is to certifty that Mr. Jonathan R. Dela Cruz was employed last October 20, 2013 up
to the Present and worked as a PRIVATE DUTY NURSE for Mr. Teck Chny, 86 years old, who was
diagnosed to have Liver Cancer, Congestive Heart Failure and COPD (Chronic Obstructive
Pulmonary Disease). He rendered 12 hours of service per day (7am – 7pm or 7pm – 7am), five to
six times in a week.

During this period, his services were found to be satisfactory in carrying out the job duties,
his responsibilities were to:

 Contract independently to render nursing care, usually to one patient, in hospital

or private home
 Administer medications, treatments, dressings plus other nursing services,
according to physicians instructions condition of patient
 Observe, evaluates and additionally records symptoms
 Apply independent emergency measures to counteract adverse developments
notifies physician of patients condition
 Direct patient in good health habits
 Give information to family in treatment of patient maintenance of healthful
environment
 Maintain equipment supplies
 Cooperate with community agencies furnishing assistance to patient
 May supervise diet when employed in private home
 May specialize in one field of nursing, like obstetrics, psychiatry, or tuberculosis

whatever purpose it may serve.
