Any change to name details must be accurately completed, as changes cannot be made afterwards.
You must provide documentary evidence of change of name.

Guest Tickets (Questions 12 & 13)
All students will be issued with 2 complimentary guest tickets, if required. Please note you do not require a
ticket for yourself.
Some complimentary extra tickets may be available, depending on numbers. Extra tickets will be allocated by
ballot and you may request up to 3 of these tickets. You will receive a letter confirming how many tickets you
have been allocated.
Ceremony Details- Collection of Tickets & Robing
Details regarding the time of the ceremony, number of guest tickets you have been allocated and robing will
be forwarded to you closer to the ceremony date.
Return of Graduation Application & Ticket Request Form
The Graduation Application & Ticket Request form must be received by the close of business on the
application closing date. Responses received after this date will be deferred until the following
conferral.

Payment can be made in person at the Finance Office or by cheque, money order (payable to the University of Ballarat) or
credit card.
