Name of work: Kaleshwaram Project, Link-II, Package-7.

Date of inspection: 09/02/2018 and 15/02/2018.

Officers Present:
1.C.Madhusudhan Reddy, DEE, QC&I Sub division no.4,Peddapally.
2.G.Narsinga Rao, DEE,Sub division no.3, Choppadandi.

Observations :

During the field inspection by Quality control team on dated:09/02/2018 and 15/02/2018 at
Km 3.9 of LMT and Km6.5 of RMT for lining, km11.100 LMT and RMT of Kaleshwaram
project Package-7, the following observations were Made.
1.The Oxygen limits observed in the said work spots were far less than the required which is
causing suffocation resulting into unhealthy working condition. Therefore, it is informed to
maintain the required Oxygen levels in the working area.

2.There is a heavy water ponding and heaps of muck in the working area, Therefore the
same shall be rectified to keep the area neat and in working condition.

3.The proper ventilation and lighting shall be provided in the entire work area.

4.The loose rock formation is noticed all along the periphery of the Tunnel. Therefore the
proper scaling shall be done to remove all loose rock fragments before erecting the Gantry,
to avoid the possible mishap.

Conclusion:
The Executive Engineer, Kaleshwaram Project Division no.4,SRSP Camp, Choppadandi is
informed to instruct the concerned to take all necessary measures for rectification and
report the compliance in this regard.
