This report shall be sent to Office on a QUARTERLY basis completed as follows:
RESULT :
This column shall indicate an assessment of condition and is graded in the following (5) categories:
GRADE 1 = GOOD CONDITION
GRADE 2 = SATISFACTORY CONDITION
GRADE 3 = SLIGHTLY BAD CONDITION
GRADE 4 = BAD CONDITION.
GRADE 5 = VERY BAD CONDITION NEEDS IMMEDIATE REPAIR
REMARK :
This column is intended for any remarks related to the mentioned item.
WORK DONE :
This column is to be used for recording work carried out on the items mentioned. The following codes are to be used for indicating the nature of the work carried out:
