I, of legal age, single, Filipino and a resident of Purok 5, Upi, Gamu,
Isabela, after having been duly sworn to in accordance with law, hereby
depose and say:

1. That I am the same person who caused this affidavit to be made;

3. That I was born on September 15, 1994 and is one of the children of
spouses;

4. That according to the best of my knowledge, I have never contracted

nor entered into any marriage contract or wedding ceremony in the
Philippines or abroad;

5. That I have not undergone any process of annulment of marriage,

declaration of nullity, divorce or legal separation in the Philippines or
abroad;

6. That my marital status remains single;

7. That I am executing this affidavit to attest the truth of the foregoing

and for whatever legal purpose it may serve.
