1. Consolidate inspection work list and submit to ENC/4 for compilation.
2. Lead TES/3 in pre shutdown discussion, Fat Rat workshop and shutdown induction. 3. Review ENC/4 JMS and make comment for an improvement. Ensure all TES/3 work lists and respective NSR are captured in ENC/4 work lists. 4. Create inspection organization chart by assigning specific role and area of responsibilities for TES/3 Inspectors. 5. Organize TES/3 pre shutdown meeting and assigned specific pre shutdown responsibilities for TES/3 Inspectors. 6. Liaise with ENC/4 Planner for DOSH inspection schedule. 7. Liaise with DOSH Kuching through TES/3 for DOSH Officer’s availability confirmation. 8. Liaise with ENC/4 for DOSH Officer’s car rental. 9. Liaise with Security for DOSH Officer’s body pass application. 10. Coordinate with Security for TES/3 Contractors security screening and body pass application. 11. Coordinate with HSE for TES/3 Contractors safety briefing and MLNG plant induction. 12. Confirm static equipment inspection, NDT, RT and ECT Contractor’s arrival timely with shutdown schedule to avoid delay arrival or standby charges. 13. Ensure static equipment, NDT, RT and ECT Contractor’s aware of latest MLNG requirements on security, safety, confine space, PTW by sending them the latest information prior to DOSH shutdown. 14. Ensure adequate and furnished portal cabin for Inspectors, DOSH Officers, NDT Technicians and ECT personnel. 15. Liaise with I-Printis to ensure adequate LAN lines for TES/3 Inspector’s computer and printer. 16. Ensure adequate supply DOSH Officer’s single piece coverall, safety shoes, safety glass, helmets, inspection tools and others consumables. 17. Manage inspection teams comprising TES/3 Inspectors, third party Inspectors, DOSH Officers, Non Destructive Testing (NDT) Technicians, Radiographic Testing (RT) Technicians and Eddy Current Testing (ECT) personnel. 18. Record inspection request (IR) forms in daily inspection progress logbook. 19. Identify an Inspector for specific equipment inspection and ensure the assigned Inspector to follow through with the assigned equipment inspection until box up. 20. Attend daily shutdown morning meeting and highlight inspection progress, major findings and areas of concern. 21. Lead TES/3 Inspectors in major repair discussion with ENC/4 and OPS/3. 22. Review DOSH report submitted by Inspectors prior to DOSH inspection.. 23. Liaise with DOSH Officers for site DOSH inspection schedule. 24. Conduct safety toolbox talk to TES/3 Inspectors and Contractors. 25. Review final SAP report with its attachments before submission to relevant parties. 26. Participate in discussion on repair of major findings or emergent work. 27. Review DOSH repair procedure before submission to DOSH for an approval. 28. Submit DOSH inspection reports and UTTM report for Certificate of Fitness (CoF) renewal. 29. Provide punch list items for shutdown Coordinator to follow up with Engineering. 30. Create shutdown inspection summary report with major findings and its recommendations and circulate to relevant departments for comment. Roles of Inspectors during for Shutdown
1. Submit inspection worklist to inspection shutdown Coordinator for compilation and
submission. 2. Timely carry out tasks and responsibilities as assigned by inspection shutdown Coordinator. 3. Involve in shutdown Fat Rat and induction workshop as and when required by inspection shutdown Coordinator. 4. Assist and participates in inspection plant stationary equipment as assigned by inspection shutdown Coordinator such as pressure vessels, column, drums, heat exchangers, boilers, furnaces, coolers, tanks, pressure relief devices, valves and piping systems throughout the plant. 5. Update DOSH shutdown Inspection Progress Chart (IPC) after an inspection has been carried out. 6. Carry out external inspection for CUI, corroded areas, support / skirt, small bores, nozzles, flange gasket face, painting and insulation condition. Ensure correct equipment tagging and name plates as registered in DOSH registered equipment in TES/3 database. 7. Carry out internal inspection and enters equipment, observes conditions and makes preliminary interpretations. 8. Fill in inspection request (IR) form and inform ENC/4 shutdown area Engineer / Supervisor regarding initial findings and recommendations. 9. Carry out final inspection (after cleaning), update IR and IPC. 10. Liaise with Non Destructive Testing (NDT) Contractor for detailed inspection by recommending appropriate NDT methods and wall thickness measurement. 11. Noting observations on crack, pits, thinning and any pattern of erosion, corrosion and other interior failure. Makes interpretations of finding and analyses for possible failure. 12. Review wall thickness measurement and rectify any abnormalities. 13. Conduct Contractor’s performance surveillance spot check to ensure their compliance to TES/3 requirements. 14. Participate in major repair discussion with Contractors, ENC/4 and OPS/3. 15. Prepares DOSH inspection report and attach UT thickness measurement (UTTM) report. 16. Prepares SAP inspection reports with detailed findings, abnormalities, recommendations and relevant attachments. 17. Submit DOSH report for inspection Coordinator review prior to DOSH submission and inspection. 18. Conduct joint site inspection with DOSH Officer and ENC/4 Supervisor. Update IPC. 19. Liaise with ENC/4 to close up DOSH Officer’s recommendations. 20. Witness pressure test or final acceptance on inspection recommendations close out. 21. Sign off IR, box up certificate and update IPC. 22. Complete final SAP report and submit to inspection Coordinator for review and approval. 23. Achieve all no outstanding inspection report in SAP system. 24. Create new SAP report for any outstanding recommendations that cannot be carried out during DOSH shutdown and submit to engineering area Custodians. 25. Witness Welder Qualification Test (WQT) and any inspection test plan (ITP) hold points. 26. Review and ensure inspection test pack compliance before signing off test pack completion.
