INSURANCE SECTION

THIS CONTRACT IS CONTINGENT UPON INSURANCE APPROVING THE WORK STATED ABOVE.

TOTAL: The PRICE is the job total on the insurance company’s SCOPE OF LOSS or ESTIMATE. This is an itemized
list of the work to be performed that is sent to the insured when the claim is approved.

AUTHORIZATION AND ASSIGNMENT OF BENEFITS:

Contract is contingent on the insurance company paying for the roof or any other claim related damage and will be
void if the insurance company denies the claim.

Your out of pocket expenses cannot exceed your deductible unless agreed upon in writing. All upgrades and/or
substitutions including hidden damages and bad wood may incur additional charges not paid by insurance company.

RISE Contractors is a roofing contractor and will do the following if necessary: an inspection, take photographs,
prepare an estimate for the insurance company, meet adjusters, submit change orders, and discuss damage.

You authorize RISE Contractors to make adjustments and settle the claim with your insurance company.

If applicable, 20% overhead and profit will be billed separately to your insurance company.

This is a contract stating that you “the customer” agree to let RISE Contractors perform the work for the total amount
that the insurance company approves for the job and that you are responsible for paying your deductible and any
depreciated funds if applicable.

You agree NOT to put RISE Contractors in a bidding war.

RISE Contractors is the contractor to do the job if the claim gets approved and the price is defined on the insurance
scope of loss you agree to give RISE Contractors a copy of your insurance Scope of Loss.

You authorize RISE Contractors to deal with your mortgage lender regarding payment disbursements.

You authorize your insurance company to make claim checks payable to RISE Contractors.
TERMS & CONDITIONS SECTION

THE CONTRACT ON THE FACE HEREOF AND ANY AGREEMENT MADE PURSUANT THERETO BETWEEN
RISE Contractors (THE “COMPANY”), AND CUSTOMER(S) WILL BE SUBJECT TO ALL APPROPRIATE LAWS,
REGULATIONS AND ORDINANCES, AND TO THE FOLLOWING SPECIAL TERMS AND CONDITIONS.
1. All proposals subject to approval of our Credit Department and Management.
2. RISE Contractors, has the right to order excess material. These materials will not be charged above the agreed
upon price. All excess material belongs to RISE Contractors.
3. Supplement paid by the Insurance Company for the additional labor and materials needed beyond the original
scope of repairs shall be paid directly to RISE Contractors.
4. All material is guaranteed as specified. All work to be completed in a workmanlike manner according to standard
practices. Any alterations or deviation from above specifications involving extra costs will be executed only upon
written orders, and will become an extra charge over and above the estimate. All agreements contingent upon strikes,
accidents, or delays beyond our control. Owner to carry fire, tornado, and other necessary insurance.
5. This Agreement constitutes the entire agreement between the parties. It may be changed only by written instrument
signed by both parties.
6. Any representation, statements, or other communications not written in this contract are agreed to be immaterial,
and not relied on by either party, and do not survive the execution of this contract.
7. If this contract is cancelled by the customer later than three (3) days from execution but prior to commencement of
work, customer shall pay to company twenty-five percent (25%) of the contract price as liquidated damages, and not
as a penalty, and the Company agrees to accept such as reasonable and just compensation for said cancellation.
Payment is due to the address listed on front.
8. RISE Contractors is not responsible for any cracked or broken driveways.
9. You agree to remove breakables from outside walls of home during installation of work
10. The Company is not responsible for any damage below the roof due to leaks by excessive wind of 60 mph, ice
dams, hail, pre-existing or future construction defects caused by storms or lack of maintenance during the period of
warranty, or tire depressions in a driveway caused by trailers or dumpsters.
11. Tarps are not considered roofing materials. By signing this agreement you understand that a tarp will not protect
your roof or prevent water from intruding into your home. We may apply a tarp to help prevent further damage to your
roof, but we cannot guarantee this prevention. Further, a tarp is used to divert water to another area on the roof. If a
tarp is used and the water is diverted, it is possible that a new leak will progress on the roof. By signing this contract
you understand that the company is not responsible for any new leak or new damage caused by applying, adjusting,
or removing a tarp on your property. The company has the right to charge the customer for any trips made to the
property for the purpose of adjusting the tarp.
11. Upon presentation of invoice, the job payment in full is immediately due. Interest rate of 1.5% per month shall
accrue beginning the day thereafter. Should RISE Contractors utilize the services of an attorney to collect amounts
due under this agreement, it shall also recover all costs of filing and releasing liens, court costs, and its reasonable
attorney’s fees incurred in collection efforts. If full payment is not made, warranty is void.
12. The dollar amount of the insurance contingency contract is the amount allowed by the Insurance Company for the
approved scope of loss plus any agreed-to upgrades and the deductible
OUR GUARANTEE SECTION

The fine print of this section:

Upon completion of work RISE Contractors guarantees work performed in this contract for a period of two years
against defects in material and workmanship. This guarantee does not extend to damage from any other cause
including, but not limited to damage from other trades, extreme wind or ice, lightning, hailstorm or other
unusual occurrences. This guarantee does not extend to the repair of any interior feature of a structure.

THERE ARE NO OTHER WARRANTIES, EITHER EXPRESSED OR IMPLIED BY RISE CONTRACTORS.

CASH PRICE & PAYMENT TERMS SECTION & SIGNATURE
