AFFIDAVIT

I, CHARLIE VARGAS TUMABIENE, of legal age, Filipino, and

a resident of Panitugan, Sulangan, having been duly sworn in
accordance with law, hereby depose and state:

1. That I am the same affiant herein;

2. That I am the second degree relative of ARLENDA TOÑION

TUMABIENE;

3. That I am the only nearest kin of ARLENDA TOÑION

TUMABIENE in Cebu City to process and receive her Birth
Certificate from the Philippine Statistics Authority (PSA);

4. That the other siblings and relatives of ARLENDA TOÑION

TUMABIENE have authorized me to be the one to process
and receive her Birth Certificate from the PSA as they are
living in Bantayan Island and cannot personally process and
receive the same;

5. That I am executing this affidavit to certify or attest to the

truth of the foregoing facts, and for whatever legal purpose
this may serve.
