The Matter of your absences has been brought to our attention. Your attendance records
for the month of July show that you were absent without leave on the following dates:
1. June 04, 2019
2. June 17, 2019
3. June 24, 2019
4. June 25, 2019
5. July 8, 2019
6. July 9, 2019
7. July 10, 2019
8. July 11, 2019
9. July 22, 2019
10. July 29, 2019
Under the Labor Code of the Philippines, habitual absenteeism constitutes gross and habitual
neglect of duties which may be a cause for suspension and or termination.
In view of the foregoing, you are hereby required to submit a written explanation to this office
within forty-eight hours (48) from receipt of this notice, stating why the company should not
impose the penalty of suspension/dismissal for the act mentioned.
Your failure to submit your written explanation within the period provided shall be construed as a
waiver of your rights to be heard and the Company shall decide on the matter based on the
available records and evidence.
Your compliance is strongly encouraged.
