WAIVER OF RIGHTS

I, MARIANO M. JAYME, JR., of legal age, single, Filipino and a resident

of Purok 6, Kalasungay, Malaybalay City, Bukidnon, after having been duly
sworn to an oath in accordance with law, hereby depose and say:

1. That I applicant for a water concession with Malaybalay City Water

District under Account No. 21-00027;

2. That for and in consideration of my love and affection I, the

undersigned, do hereby WAIVE, TRANSFER AND CONVEY all my
rights, interests and obligations over above-mentioned concession;

3. That this waiver is being executed in favor of my mother

ASUNCION M. JAYME, of legal age, widow, and a resident of Purok
6, Kalasungay, Malaybalay City, Bukidnon;

4. That this affidavit is taken for the purpose of informing all the
concerned persons and agencies especially the Malaybalay City
Water District about the foregoing facts;

5. That I hereby manifest, affirm, swear and testify to the correctness

of the foregoing.

6. That I am voluntarily executing this affidavit for whatever legal

purpose/s it may best serve.
