AFFIDAVIT OF LOSS
I, PAOLO NIKKO GARCIA RUSTE, of legal age, Filipino, single, and residing at
Talungun, San Roque, Zamboanga City, Philippines, after having been duly sworn
to in accordance with law, hereby depose and state:

That I am the owner of a driver license issued by the Land Transportation

Office, Zamboanga City;

That sometime March 2019, when I was about to present a valid ID at an

establishment with the intention of presenting my driver’s license, to my surprise,
the same was not inside my wallet. I tried to recall as to my whereabouts within
the day before that time; however, I my prior activities did not prompted or
required me to present my driver’s license;

That, I exerted effort to recover the same by going home and checking my
personal belongings; however to no avail. Hence, I now consider it lost and beyond
point of recovery however to no avail; and

That I am executing this affidavit to attest to the veracity of the foregoing

facts and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand 19th day of July 2019 in
