1. Copy of Official Receipt of payment for Certificate of Registration and Tax Exemption Renewal Fee of
Php2,000.00, Business License Sticker Fee of Php200.00
2. SBMA Approved Notarized Lease/Sublease Agreement inside the SBFZ

Clearances issued by other SBMA Departments


1. Copy of Locational Clearance issued by the Planning and Development Office.
2. Copy of the Fire and Safety Clearance issued by the SBMA Fire Department
3. Sanitary Clearance (as maybe required) issued by the Public Health and Safety Department

Other Requirements: (To be submitted prior to release of CR/CRTE)


1. Latest Audited Financial Statement (AFS)
2. Latest Annual Income Tax Return (ITR)
3. Updated SEC General Information Sheet/SEC Certification (for corporation/partnership)
4. Updated Insurance Policy stating SBMA as the beneficiary (for direct lessees only)
