2. That I am intending to apply as CANDIDATE SOLDIER in the Armed

Forces of the Philippines, Philippine Army, and when I inquire about the
necessary documents to facilitate my application, one of the
requirements is an affidavit of my legal beneficiary;

3. That I am designating my parents CARMELITA ALER-URBANO and

MARDONIO PAYAO URBANO as my legal beneficiaries, relative to my
application with the Armed Forces of the Philippines;

4. That I am executing this to attest to the truthfulness and veracity of the

Forces of the Philippines, Philippine Army, and when I inquire about the
necessary documents to facilitate my application, one of the
requirements is an affidavit of being single;

2. That I am single as to date;

3. That I never contracted marriage to anyone;

4. That I have no dependent/child;

5. That I am executing this affidavit to attest to the truthfulness and

veracity of the foregoing statement for whatever legal purposes this
