I. Please accomplish this Gate Pass in three (3) copies

INGRESS GATE PASS: Exhibit items/materials should be listed in the gate pass form to
be approved by the Silver Lotus office prior to entry
Gate Pass Distribution during INGRESS:
o 1 COPY – Silver Lotus
o 1 COPY – Building Manager
o 1 COPY - Supplier – should be kept until Egress
II. Items will only be allowed entry if covered by a properly accomplished Gate Pass.
III. Do not lose this Gate Pass as this will also be used in moving out the abovementioned
items.
EGRESS GATE PASS: 1 copy from Ingress gate pass signed by Building Manager prior to
exit
Gate Pass Distribution during EGRESS:
o 1 COPY – Supplier’s Copy – to be surrendered to the Building Manager
IV. PROPER DRESS CODE
We will not allow entry to staff wearing sando, sleeveless shirts, ripped shirts/jeans,
shorts, flipflops/slippers and any footwear exposing the toenails.
V. Strictly NO ID, NO ENTRY.
Supplier staff are required to surrender their company ID in exchange for the Silver
Lotus Supplier’s Pass.
