PETITION

COMES NOW, Atty. Victor Baltazar Salting, of legal age, and

to this Honorable Court, respectfully alleges:

1. That he is a citizen of the Philippines, over twenty-one

(21) years of age having been born on 20 APRIL 1993;

2. That he is currently residing at Jasper St., Rufina III

Subd., Brgy. Malasin, San Jose City, Nueva Ecija, and his law
office is located at Jasper St., Rufina III Subd., Brgy. Malasin,
San Jose City, Nueva Ecija;

3. That he has satisfactorily completed the required course

in law in PHINMA Araullo University in Brgy. Bitas, Cabanatuan
City, Nueva Ecija;

4. That he has passed the Bar Examination on November

2018, and has since been admitted to practice law in the
Philippines, with Attorneys Roll No. 73120, as shown by the
Certificate of Membership in the Philippine Bar, a copy of which
is herein attached and marked as ANNEX “A”;

5. He has no derogatory records and is not charged with

any offense as certified by the National Bureau of Investigation,
a copy of which is hereto attached as ANNEX “B”.

6. That in support of his petition, the petitioner has

attached herein the following:

(a) Certification that he has no Administrative or

Disbarment Case issued by the Office of the Bar

 Confidant, herein attached as ANNEX “C” and made
integral part hereof;

(b) Certification of Good Moral Character by at least two

(2) executive officers of the Integrated Bar of the
Philippines, herein attached as ANNEX “D” and made
integral part hereof;

(c) Three (3) unretouched passport-size colored

photographs with light background within thirty (30)
days prior to this application, herein attached as
ANNEX”E” and made integral part hereof;

(d) Three (3) specimens of his signature, and true

impression of his dry seal, as ANNEX “F” and made
integral part hereof.

7. That he desires to be commissioned as Notary Public for

and in the City of San Jose, Nueva Ecija, and that he possesses
all the qualifications and none of the disqualifications for the said
office.

WHEREFORE, it is most respectfully prayed of this

Honorable Court that after hearing, an order be issued granting
the instant petition as Notary Public in the City of San Jose,
Nueva Ecija.

Petitioner likewise prays for such other and further relief as

may be deemed just and equitable in the premises.

residing at Jasper St., Rufina III Subd., Brgy. Malasin, San Jose City,
Nueva Ecija, after being sworn in accordance with law, deposes and says
that:

1. I am the Petitioner in the above-entitled petition;

2. I have caused the preparation and filing of the foregoing petition;

3. I have read and understood the allegations therein contained and

the same are true and correct of my own knowledge and based on
authentic records;

4. I have not heretofore commenced or filed any other action

involving the same issues in any court, tribunal or agency; that of my
own knowledge, no such action is pending in any court, tribunal or
agency; that should I hereafter learn of any such pending action, I
undertake to inform this Honorable Court of such fact within five (5) days
therefrom.
