A STEP BY STEP PROCESS ON HOW TO OPEN A PASSBOOK SAVINGS

ACCOUNT.

 I went to a cooperative named Southern Leyte Employment

MultiPurpose Cooperative (SLEMCOOP).

 I filled out a paper given to me with the sufficient information they

needed and signed it.

 I handed to them the requirements: the initial deposit of 100.00, 2

copies of most recent 2 x 2 I.D. picture, and a photocopy of my school
I.D.

 I was presented with the receipt and the account summary of my

deposit.

SPECIAL NOTE: The actual passbook was given the following week.
