Interventions:
1. Weekly check-in on Monday 6th hour, process with case manager. Started 9/9.
2. Intervention #2 starting after MEA (10/21) through Thanksgiving break (11/26). Social
Skills training 5th hour with Meier (2x/week for 30 min)- Zones of Regulation and
Character Strong . → 2nd Intervention follow-up ​ ​IEP meeting will be set up for
11/18/2019. If parents don’t attend 11/18/2019, then new notice of meeting will be sent
home nad IEP will be held on 12/2/2019

→ Tara will set up IM4 system, get teachers on board (Fisher, Thompson, Quach); will explain
data collection

follow directive​. JM was given multiple prompts to stop talking during lesson, he kept talking
and got loud. It became unmanageable, so he was asked to leave.

9/18 9:10am Referred by Christiansen, incident location classroom, incident type ​repeated
refusal to follow directive​. Student w ​ as​ sent to 143. Date contacted home 9/10 voicemail.
Description- Teacher asked JM to turn around and he said she was not supposed to talk to him.
He said, “get away, I can multi-task!” He got agitated and banged on the chromebook. Teacher
asked JM to step in the hallway and he said he wasn’t talking to her. She asked again and he
refused. He did not leave at first, but then did. Resolved w/ Redepenning. Met with student and
contacted home.

9/20 9:15am Referred by Christiansen, incident location classroom, incident type ​repeated
refusal to follow directive​. I asked JM to talk in the hall. He said I can’t be around him. When
teacher brought him to the hall, he said teacher only talks to him and that she is racist.

9/26 9:45am Referred by Christiansen, incident location classroom, incident type ​threatening​.
Student ​was​ sent to 143. Date contacted home 9/10 voicemail. Description- JM was messing
around on the computer. I looked at his computer and he said, “if you close out my game,
there’s going to be a problem.” Resolved w/ Redepenning. Met with student.

9/27 8:05am Referred by Christiansen, incident location hall, incident type ​harassment​ Student
not​ sent to 143. Date contact home 9/10 voicemail. Description- Walked by my room and called
me a bitch. Resolved w/ Redepenning. Met with student 9/27, student has been transferred out
of class.

9/27 12:37pm Referred by Fisher, incident location classroom, incident type ​repeated refusal
to follow directive​. Student ​was​ sent to 143. Description- JM kept interrupting me as I was
delivering the lesson. He was given multiple opportunities to correct his behavior. He kept
interrupting and he was asked to leave.

10/1 8:40am Referred by Fisher, incident location classroom, incident type ​truancy, repeated
​ as​ sent to 143. Description- JM wanted to talk with his
refusal to follow directive​. Student w
case manager. I called, but she didn’t answer. I left her an email asking her to contact me
when/if she becomes available. JM announced that he was going to her room. I said no. I called
and she didn't answer. He walked out without permission. Resolved w/ Redepenning. Met with
student on 10/1, resolution PAWS.

10/3 8:40am Referred by Fisher, incident location classroom, incident type ​truancy​. Student
was not​ sent to 143. Description- JM walked out of class without permission. He was asked
multiple times to return before the decision was made to document the behavior.

10/7 10/8 4th hour by Thompson, incident location classroom, incident type ​repeated refusal to
follow directive​. Description- Student refused to do work and continuously disrupted the
learning environment. He also said, “I hate this F*** class” and was yelling at me across the
room about not having materials. → student was asked to leave both days. → on 10/8, a
paraprofessional had some interaction with him in the hallway.

10/8 12:35 Referred by Fisher, incident location classroom, incident type ​repeated refusal to
follow directive. ​Student ​was​ sent to 143. Description- JM repeatedly interrupted me when I
was delivering the lesson in Study SKills class. He was given multiple opportunities to change
his behavior. He became verbally aggressive and also told me to “fuck off.”

10/9 6th hour Referred by Imholte, incident location classroom, incident type
bullying/harassment ​and ​repeated refusal to follow directive​. Description- He groped
another student as she was sitting down working by sliding his hand between her legs.
