Part 1

Give your insights on the case and considering that you are a second-year
medical student and a future health professional, consider the classroom
discussions and work on the following items:

A. Which of the health problems presented in the case is core and priority
for health program development? Explain your choice.

Considering the socioeconomic and environmental standpoint of the family as


major precipitating factors of the health outcomes in the presented case, the core
and priority health problem here is the lack of proper health education and this
should be the focus of the health program development. Low educational
attainment may be rooted from poverty and this primarily affect their behavior
towards health. Proper knowledge on child nutrition, such as proper milk
preparation, and sanitation, as in proper cleaning of milk bottles, are evidently
lacking in the case. In addition, there is also a lot of misconceptions regarding some
medically important factors. There is wrong notion regarding worms as good
agents for digestion, and vaccination as fever-causing and therefore not good for
their child. Developing health education programs such as lectures, seminars and
other community programs to increase awareness and knowledge of health-related
issues may help resolve this kind of problem. Secondarily, due lack of government
funding, out-of-pocket expenditure on medications that are supposedly free in our
primary health centers contributes largely to the family’s financial burden, and
ultimately leading to the most unfortunate medical outcome which is death.

The Double Burden of Disease

The coexistence of communicable and non-communicable diseases (NCDs),


also termed as the double burden of disease, has been impacting populations
significantly across the world. According to WHO, of the five leading causes of
morbidity in the Philippines, four are communicable diseases (diarrhea, bronchitis,
pneumonias and influenza), and the fifth is hypertension. The prevalence of
communicable diseases is still very high, while that of non-communicable diseases is
increasing and is predicted to continue to do so. This places a great toll on our health
and economy as a nation. As a second-year medical student, the best thing I can do
is educate people on things that promote health seeking behavior. This includes
proper health practices, good and healthy lifestyle changes including diet. This is very
relevant today especially now that there is increasing stigma against having
vaccination due to the recent Dengvaxia incident. Proper education of the people could
eradicate these wrong notions. I believe that everything starts by knowing, therefore,
education is the key to sensitizing people that leads to a changed behavior.
