Fracturing operation at well pad shall be carried out under the direct personal
1 supervision of a competent person holding degree in Petroleum Engineering from a
recognized institute and authorized in writing for then purpose by the manager.
Only brine solution shall be used for fracturing operations in formations. Under no
2 circumstances diesel or any inflammable material shall be used for fracturing
operations.
Only trained and experienced worker shall be engaged in carrying out hydraulic
3 fracturing operations. Workers shall under take only such work for which they are
trained
All pipes, valves, flanges, other fittings and accessories shall confirm to API, ASA, ISI or
equivalent specifications. Whenever there Is any doubt about the soundness of any

material, it shall not be used. Material already used shall not be re-used unless it has
been thoroughly examined and found to be in good working order.
(I) Each oil well installation shall be provided with built-in emergency shutdown
system.
(ii) Each oil well installation shall be provided with safety relief system as per API RP
4.1
521 and OISD standard-106.
(iii) Each oil well installation shall be provided with fire protection and fighting systems
as per OISD standard-189.
Adequate and suitable instruments shall be provided for recording pressure, flow

level etc. at suitable points.
Pressure being the most critical component shall have following control other than
manual control to maintain a pressure safety margin and prevent damage to system
integrity including high pressure piping, equipment, personnel and environment.

i. Electronic pump kick-out and burst discs on each pump and pressure relief valve on

the main line shall be incorporated as over pressure protection in high-pressure
piping.
ii. All connecting piping shall be tested for leaks by pressurizing to operating pressure.
iii. Temporary piping from the chemicals tanks additive containers to the blender shall
also be subjected to leaks test.
Pressure relief devices shall be set to discharge at a pressure less than the rated
working pressure of the weakest component (pump, piping, hose, or fitting, etc.) of

the
system that the devices protect.
Valve positions and relief settings shall be checked and high pressure lines from the

pump trucks to the wellhead shall be inspected before starting work.
Pump and discharge lines shall be subjected to test at a pressure not less than 1.5
5.4 times the maximum working pressure at least once in six months, but not to exceed
the rated working pressure of the component with the lowest rated working pressure.
Key treatment parameters (such as pumping rates, slurry density and treating
pressure) shall be monitored constantly and compared with expected values from the
hydraulic fracturing design. Any significant deviations observed related to uncertainty

in reservoir geology or in equipment performance shall be investigated. The well site
Petroleum Engineer shall have responsibility and authority to cease work until a
review of the activity can be concluded and it is established that activity can be
resumed safely. Any such stoppage and resumption of work shall forthwith be
intimated to this Directorate.

It shall be ensured that all emergency shutdown and switches are in proper operating
condition and that all members of the Well Services are fully trained in emergency

equipment shutdown procedures. ESD system on frac flow back system shall be used
and tested at regular intervals.
The area of operation shall be clearly demarcated and completely barricaded. The
7 well/installation shall be securely fenced so as to prevent the ingress of any person
except through manned gate.
Only those personnel required for the fracturing operation shall be allowed in the area
7.1
when the operation is in progress.
All work persons in the area shall be provided with brightly colored florescent jackets
7.2
or reflective vests.
Adequate illumination by way of suitable flood lights mounted on suitable tower shall
be provided so that work beyond daylight hours can be carried out properly.
Illumination at the cellars and work area on the pads shall not be less than 70 LUX.

Illumination shall be measured at least once in a week and a record of the same shall
be maintained. Every persons employed in the fracturing operation shall be provided
with efficient lamp of approved type.
Alternative arrangement shall also be made for continuous illumination in case of
8.1 power
failure.
All electric lamps in the area shall be enclosed in flame proof enclosures. All electrical
fittings and accessories in the installation, cables and all apparatus including portable

and transportable apparatus shall conform to the provisions of Regulation 110 of the
Central Electricity Authority Regulations, 2010.
All equipment shall be electrically connected with the earth in efficient manner, which
9.1 shall be periodically tested for its effectiveness, and a record of the same shall be
maintained in a bound paged book.
Advanced early streamer emission type lightning protection system as specified in

DGMS Tech Circular No. 7/2008 shall be provided.
No hydraulic fracturing operation shall be carried out during approach of storm or
10.1 lightning. Hydraulic fracturing operation shall not be resumed till the storm has
passed.
Sufficient space shall be maintained between equipment and materials and it shall be
11 ensured that workers stay clear so that they are not crushed by unexpected
movement of equipment and/or material.
Traffic rules shall be framed and strictly followed. Movement of heavy vehicles and

Equipment transfer shall be strictly prohibited during night.
Only trained and qualified operators shall be allowed to operate mobile lifting and
13
hoisting equipment.
To avoid inherent risks during refuelling of equipment in high pressure and high
14 temperature zones, all equipment shall be fuelled during day light hours. Refuelling
shall not be done in night hours except during downtime between jobs or stages.
Neutralizer for acid and Material Safety Data Sheet for each additive/chemicals used
shall be readily available at the job site. SOPs/standing orders for proper storage and

handling of chemicals and spill clean-up procedures shall be prepared and posted at
conspicuous places.
Persons involved in handling of chemicals/hazardous materials shall be provided with
suitable protective equipment including respiratory protective equipment, eye
16
protectors, gloves and aprons. Suitable protective outer clothing for use in rain and
extreme weather conditions shall also be provided.
Waste or refuse from the fracturing operations shall not be permitted to create a
hazard to public health and safety and shall not be allowed to run into or contaminate

any fresh water structure or body of water or to remain in a place from which it might
contaminate any fresh water or body of water.
Appropriate detectors for H2S gas shall be installed on pit. Hand held gas detectors
shall also be provided to persons. Frac flow back shall be suspended if at any time H2S
gas at the pit crosses 20 ppm. Operation shall not be resumed till H2S gas

concentration comes down to less than 20 ppm. Every such occurrence shall be
recorded in a bound paged book kept for the purpose and counter signed by the
Petroleum Engineer.
Inflammable gases may be present as the formations are fractured. Suitable
instruments for testing for presence of inflammable gas/petroleum vapour shall be

provided. It shall be ensured that LEL meter is provided and continuously operating in
the cellar area.
Water ring main with adequate storage of water at site, pump feeding hydrants and
water monitors shall be provided and maintained. Fire fighting equipment shall be
provided to every part of the installation. The water pumps provided to feed the
20 hydrant line must have the capacity to develop adequate pressure for generating
proper water jet and foam. Pumps with two independent source of power shall be
provided. In addition, fire tenders shall standby while pumping and handling of volatile
substance is in progress.
Fire fighting equipment shall be inspected and tested once at least in every 30 days by
a competent person. The report of every such inspection and test shall be recorded in

a bound paged book and shall be signed and dated by the person making such
inspection and test.
Approach roads to all the equipment/installation shall be such that fire tenders can

reach them in all weather.
Fire alarm system shall be provided so that fire station can be immediately informed
20.3
in case of any occurrence of fire.
Contingency plan for fire and emergency shall be prepared. Organization for dealing
with fire and emergency shall be established by the manager in each mine. List of key
persons with name, designation and mobile & landline telephone numbers to be

contacted in case of fire/emergency shall be displayed conspicuously at strategic and
relevant places, installations and offices. Such list shall be made up to date once in
every quarter.
Efficient telephonic communication shall be provided and maintained in working order
at all times between the installation and mine officials. Second means of
21 communications also shall be established. On well pads with several wells under
production or injection process, a proper two level communication shall be
maintained between the Petroleum Engineer and production operator.
Mock drill on Emergency Response will be conducted at night once in every three

months. Record of such mock drill shall be maintained in a bound paged register.
Occupants/Owners of houses/dwellings/buildings or other structures of public
23 authority concerned if any shall be indemnified for damages to their properties or
injuries to them or to other persons due to operations conducted under this approval.
All other relevant provisions of the Oil Mines Regulation, 1984 shall be strictly

complied with.
(a) In the event of any change in the circumstances connected with this approval that
is likely to endanger mine or persons employed therein , all operation shall
immediately be stopped forthwith and this Directorate intimated about the same. The
operations shall not be resumed without a written permission from this Directorate.
(b) If at any time any of the conditions subject to which this approval has been granted
is violated or not complied with, this approval shall be deemed to have been revoked
with immediately effect.

(c) The above approval may be modified, amended or withdrawn at any time, if
considered necessary in the interest of safety.
(d) This Directorate shall be informed as soon as operations are commenced in
accordance with the above approval. Intimation about completion of the operations
should also be sent promptly and in any case not later than one month thereof.
(e) This approval shall remain valid up to three years from the date of issue of this
letter.
