Dear Ma’am,

I wish to formally notify you that I am resigning from my position as QC Bulk Analyst with
Siegranz Chemworks that will take effect on May 3, 2018. I will provide all necessary support to
transfer my duties to my replacement. I will render my SIL on April 18-24 and my VL on April 25-
28. All my remaining days until May 3 that will not be covered with my earned leaves will be
without pay as agreed upon with Ms. Saligo.

Working at the company was a great learning experience for me, yet I am having difficulty and
notice that my health is declining due shifting duty schedule. Also I have decided to leave to
continue my trainings and pursue my review to take this year’s two licensure examinations.
