1. I Submit that I have read the G.O.Ms.51, Tribal Welfare (LTR) Department, dated

01.09.2017 issued by the Government of Telangana for the purpose of Chief Minister

Scheduled Tribe Entrepreneurship & Innovation Scheme and I am fully aware of the eligibility

criteria mentioned in the above G.O.

2. I declare that, I am neither in employment of any Central or State Government, nor

employed with any Public Institutions.

3. I swear that, I have not availed of any other scheme under Central/ State/ any Public

Undertakings nor I am due to pay any amount/ interest to any of the institutions.

4. I swear that, none of my family members have been benefited under the Chief
Minister Scheduled Tribe Entrepreneurships & Innovation Scheme (CMSTEI) earlier.

5. I swear that, neither me, nor any of my family members are involved in any of the

crimes and there are no criminal cases pending before any Police Station or Courts.

6. I swear that, the unit/ scheme sanctioned will always subjected to inspection by Bank

Authorities/Government/its representatives, and I will neither transfer nor lease nor sell my

unit at any point of time and it will be maintained as per the terms and conditions of the

scheme sanctioned.

7. I swear that, I will ensure that a minimum of 50% employees form ST Community are

engaged mandatorily in the unit/scheme sanctioned as per the guidelines of Government.
