I (K.V.Subba Rao) submit that I have been working as Accounts Officer, O/o

DSW., AP., Amaravathi. I am included in the panel year 2017-18 for getting promotion

from AO/AD to CAO/DD.

Further, I submit that, my health is not well. I am suffering from Diabetic and

Hypertension patient and undergoing medical treatment at Vijayawada since long

time. In this regard I have to stay in Vijayawada for my medical treatment. The doctor

advised me that don’t make journeys and stay with the assistance of the blood

relatives. Present I am at the edge of retirement and life.

Hence, I request the kind officer, may pl. be posted at Vijayawada Accounts

branches in head of the departments so for further continuing of my medical

treatment. I and my family members pray the kind officer may pl. be gifted further life

to me under medical grounds and sympathetic grounds. I and my family members

shall ever grateful to you sir.
