SUBJECT: OPTING FOR CASH SETTLEMENT/CASH CLAIM

Greetings. This letter is with regard to my insurance claim for the damage sustained to
my motor vehicle, a TOYOTA VIOS, with conduction YX1816, which was insured from your good
company. Based on the oral discussion with Chin L. Canita when I visited your office last February
3, 2020, I submitted the necessary documents for making an insurance claim. I also mentioned
during the said date that I opted for a cash settlement from your good office. I received a text
message on February 4, 2020, from Jackie of Connext Insurance Agency, that my claim has been
submitted. I received a phone call from the same person on February 14, 2020 that my insurance
claim has been approved, which was further confirmed by Ms. Ann Velo, an agent of Toyota via
a text message on the same date. However, with the advice of Ms. Jackie, I am writing this letter
since I would like to reiterate that I am opting for a cash settlement or a cash claim due to the
following reasons, to wit:

I authorized my son to use the aforementioned vehicle for his personal use, and he uses
the same to travel to school and back in our residence;

It is of common knowledge that the duration of the work necessary to repair the said
vehicle in the Toyota Service Center would entail a lengthy period, which would otherwise
deprive me of my use of the same vehicle for such period than that necessary;

The authorized repair shop of your insurance company is located in Paranaque City, which
is of a lengthy distance, considering that I reside in Pasig City;

There are also a number of reputable auto repair shops within my vicinity, to which I could
avail of the necessary services for the repair of the said vehicle without costing me
precious time and costs of commute;

For the above-mentioned reasons, I respectfully request for a cash settlement.
