COMPLIANCE

1. I, Project Supervisor/Acting General Manager of MASBATE ELECTRIC

COOPERATIVE, INC (MASELCO), with office address at Pinamarbuhan, Mobo, Masbate by
these presents, after having been duly sworn to in accordance with law, do hereby, depose and
state THAT;

2. MASELCO, thru the Legal Counsel and with the conformity of HR Head, Victoria
Reyes and the Officer-In-Charge, Mark John Montilla, submitted unto this Honorable Office the
Supplemental Proof of Compliance dated 28 January 2020 and the motion to submit the Annual
Physical Medical Examinations Result not later than 28 February 2020;

3. In view of the Supplemental Motion dated 28 January 2020, I respectfully would

like to submit the attached Annual Physical Medical Examinations Result;

4. Herein Respondent-employer, move that herein Compliance be ACCEPTED and

relieve MASELCO from payment of any penalties/fines relative to the Compliance Order dated 15
November 2019;

5. I am executing this Compliance and to attest to the truth of the foregoing

statements and for all legal intents and purposes it may serve.
