SWORN STATEMENT

I, CARMEN S. MATURAN, Filipino, of legal age, widow and a resident of

Barangay Poblacion 1, Dumaguete City, Negros Oriental, Philippines, after being duly
sworn to in accordance with law, hereby depose and state:

1. That I am the awardee and holder of Stall No. 14, Building No. 1, Fish
Section, Dumaguete City Public Market Complex;

2. That for the reason of my old age “69 years old”, there are times that I
cannot personally attend my business in the aforementioned stall;

3. That in times of my absence, I hereby authorize my HELPER, Ms. DIVINA

NOAY whose picture is hereto attached, to act in my behalf to sell my goods
and merchandise in the subject stall;

4. That the person I employ as HELPER shall, under no circumstances, be

person with the stall holder has any commercial relation or transaction
(Res. #60, Ordinance No. 16, series of 1988, Article No. 10, Section No. 4);

5. That at all times have my picture and that of my authorized helper

conveniently frame and hung up conspicuously in the stall.

6. That violation on my part or on the part of my helper of the foregoing market

rules and regulations shall be sufficient cause for the market authorities to
cancel my leased Contract (Resolution No. 60, Ordinance No. 16, series of
1988, Article No. 10 Section No. 1)

7. That I am executing this affidavit to attest the truth and veracity of the
foregoing declarations and for whatever purposes this may lawfully serve.
