5. Objectives
1. To propose the new technique for forward and downward movement of upper incisors in Class
III growing patients.
2. To investigate dentoalveolar changes after the application of the light-force partial fixed
appliance in Class III growing patients.
3. To examine the change of the alveolar bone quantity of the upper anterior dentoalveolar area
after the application of the light-force partial fixed appliance in Class III growing patients.

6. Background information / Significance of Research Project


In class III growing patients, maxillary protraction facemask is common orthopedic treatment
but the effect of this appliance is proclination of upper incisors (that lead to poor esthetic and prone to
labial bone resorption) and clockwise rotation of mandible rather than orthopedic effect on maxilla.
Moreover, uncomfortable, prolonged treatment time and poor patient’s cooperation may, in turn,
negatively influence the treatment effect. Regarding a recent concept, light forces applied to teeth
may affect both displacement of both tooth and supporting alveolar bone and safe for periodontium.
So, there is probably advantage in using the alternative treatment for class III growing patients with the
light force 2x4 fixed appliance with advancing loop and short light class III elastics that simple used in
clinic, less discomfort, less cooperation need and safe to periodontium used for forward and
downward upper anterior dentoalveolar movement as direction of normal growth.

7. Research Boundaries and Methodology


Class III malocclusion growing patients (12 samples) with anterior crossbite or edge to edge
incisor relationship in centric occlusion on 2 or more incisors and , from treatment plan, the upper
incisors need to be moved forward and downward will be treated with 2x4 pre-adjusted edgewise
fixed appliance (partial fixed appliance). Upper anterior segment and Lower teeth will be level and
align until 0.018”x0.022” stainless steel wire. After that, the 5x3 mm advancing loops into
0.016”(0.4mm) diameter TMA round wire (Ormco, 3M) will be added mesial to the permanent first
molars tube that place the anterior segment of the wire 1 mm labially to incisor bracket slot before
overlay ligating on main archwire and hooks for class III elastic will be bent at primary first molar or
first premolar positions for short light class III elastics (3/16”2oz or 56 g) to move upper dentoalveolar
forward and downward. The light force 70-100 g produced from this appliance is calculated from pilot
study that test the force from advancing loop 0.016”(0.4mm) diameter TMA round wire by universal
testing machine. CT scan of upper incisors and lateral cephalography will be done before using
advancing loop with elastic and after anterior crossbite correcting 6 month. Alveolar bone width (from
CT scan) and position of upper incisors (from lateral cephalography) will be measured to evaluate
treatment effects.
