AGENCY AGREEMENT WITH UNDERTAKING

KNOW ALL MEN BY THESE PRESENTS:

I, JOSEFINA QUIJANO AGUAS, a PVAO pensioner, of legal age, single/married, widow/

widower, Filipino citizen, with residence and postal address at 114 Purok 2 San Jose,
Floridablanca, Pampanga, by virtue of these presents, have named and designated as I hereby
name and designate son/daughter/sister/brother etc OSCAR Q. AGUAS, also of legal age,
single/married, Filipino, as my authorized representative to possess, hold, and/or use my
Automated Teller Machine (ATM) card, which was issued for my PVAO Pension Account thru
which my pension is remitted.

I, OSCAR Q. AGUAS, with residence as stated above, do hereby manifest my ACCEPTANCE

of this Agency Agreement and thus guarantee and undertake to exercise my authority as such
agent with prudence and to the best interest of my principal. Further, I also undertake to pay
and/or reimburse any third person who may be prejudiced or damaged for any:

“Withdrawal from the pension account of JOSEFINA QUIJANO AGUAS, my principal, who
entrusted to me his / her Automated Teller Machine (ATM) card”.

I also commit and guarantee that if something happened to said principal affecting the
authority given to me, I will immediately inform PVAO of the same and that failure on my part to
inform PVAO will constitute a violation of this Agency Agreement.

In this regard, I commit and undertake to indemnify any person who will be prejudiced or
damaged as a consequence of said withdrawal, in the amount equivalent to the total money
withdrawn plus reasonable interests and charges thereof.
