I, ANA ROSE I RENTOR, a Filipino of legal age, a resident of Uling, City of

Naga, Cebu, a bona fide member of the Philippine National Police with the position of
Administrative Aide IV and presently assigned at Regional Headquarters Support
Group, after having duly sworn to in accordance with law, do hereby depose and says
that:

1. Pursuant to this affidavit, I am confirming that the following are my legal

of my DLB are original copies obtained from Philippine Statistics Authority; and

4. I hold myself criminally and administratively liable for perjury and dishonesty
should any statements made herein/ documents submitted herewith are
inaccurate or later found to be false and in violation of law.

AFFIDAVIT

I SPO3 Wenceslao B Salimabangon, 47 years old, married and resident of Sitio

Bagalnga, Flores, Catmon, Cebu after having been duly sworn to in accordance with
law do hereby depose and say;

1. That I am presently assigned with Regional Headquarter s Support Group

designated as Chief Clerk and one of my function is to check the attendance of
personnel assigned in our office during Daily Personnel Accounting Report;

2. That at around 7:30 o’clock in the morning a formation held at Quadrangle of

this PRO7 for a Daily Personnel Accounting Report and during the checking of
attendance PO3 Arvin O Hernandez was present on that time.

3. That at around 8:00 o’clock in the morning more or less our Regional Director
PCSUPT TEBOLD SINAS arrived and made instruction that the RHSG and
RPHAU Personnel will remain for dissemination and it was notice that it was
unannounced Drug Test.

4. That I myself follow the lawful orders of the RD as part of the internal
cleansing of the PNP organization and submit myself voluntarily for a drug test
and submit my urine sample to the personnel of RDs office.

5. That right after my submission of urine sample I proceed to Base Police office
to pick-up the DPAR of Base Police Personnel that was checked by SPO3
Raganas, a Chief Clerk of Base Police. In a moment of our conversation PO1
Hilarion Ramos arrived and asked in a Cebuano dialect “ Chief Salimbangon
nakakita ka ni Hernandez? Nisibat man daw sa drug test” and I replied to him in
same dialect “wala ko masayod og asa siya”
