EXPLANATION LETTER

I, WINELFREDO I. ROSALES, of legal age, married, Filipino citizen, with

residence and postal address at Rosal St. Don Felipe Subd. Brgy Mendiola, Siniloan,
Laguna, after being sworn in accordance with law, do hereby depose and state the
following:

1. That I am working as a Responder/Driver under job order status at the

Municipal Disaster Risk Reduction Management Office of the municipality of
Siniloan, Laguna;
2. That an order was issued by the Office of the Mayor of the said municipality
requiring all employees and job order personnel to attend the flag raising of
24 February 2020;
3. That I was not able to attend the said flag raising ceremony;
4. That on the 24th of February 2020 my brother in law was confined at Eastern
Laguna Medical Hospital due to a vehicular accident;
5. That after I got out of work at 7AM on the day of the said flag raising
ceremony, I proceeded straight to Eastern Laguna Medical Hospital and
looked after my brother in law and tended to his needs while he was confined
at the said hospital;
6. That said predicament was the direct reason of my failure to attend said flag
raising ceremony and ultimately, failure to heed the aforementioned order of
the Office of the Mayor of the municipality of Siniloan, Laguna;
7. That I execute this affidavit to attest to the veracity of the foregoing
statements and to be used for whatever legal purposes it may serve.

To attest to the truthfulness of the statements written above, I hereby affix my signature
this 28th day of February, 2020 at Siniloan, Laguna.
