A. Nursing Process

1. Client's Condition

On Monday February 10 2020 a client named Nn. Galuh is being treated in

Melati room, Jalasenastri Hospital Bandung. The patient suffers from
stomachache, nausea, gag, and headache. Based on the doctor’s medical
examination, the patient is diagnosed with typhus abdominalis. It is caused by
salmonella typhi bacteria which might be influenced by bad dietary habit,
environmental hygiene, and unhealthy food.

2. Special purpose

The client knows and understands about balanced nuritions to prevent herself
from typhus abdominalis.

3. Nursing action

a. Topic : Educating the client with typhus abdominalis about balanced

nutritions (the importance of good dietary habit: eat only healthy food
3 times a day)

b. Method: Personal education (focus group discussion)

c. Media : Interpersonal communication channel

B. Communication Strategies in Implementing Nursing Actions

The clock shows 7 a.m. The nurse who are in charge of morning shift
have entered the nurses’s room and check the patients’ medical record. Then
they will immediately observe the improvement of patients who are receiving
medical treatment.
A nurse named Nur comes into a client's room

1. Therapeutic greeting

P : “Good morning ma’am and sweet child”

K : “Good morning nurse”

I : “Good morning”

P : “Let me introduce my self. My name is Yulia, I am the nurse in charge of

today's morning shift. I am going to do some rechecks. First of all, the
patient's name is Galuh. Am I right?

K : “Yes, right nurse”

P : “Excuse me, would you let me check your patient's bracelet?”

K : “Okay”

P: "Would you please tell me your name and date of birth?”

K : “I’m Galuh Dini Kumalasari and my date of birth is December, 06 2006”

P: “Yes, it’s right, so Galuh, how are you? Do you feel better today?”

K : “I still feel the pain in my stomach and my mouth is bitter”

P : “Okay, what about galuh’s appetite recently?”

I : “ her appetite has decreased since she gets sick"

P : “It's better for us to maintain her appetite, ma'am. Try to ask her to eat
little but often.”
K : “I will, thank you for your suggestion.”
 P : “Galuh, excuse me I will examine your vital signs including blood
pressure, pulse, temperature, and breathing. are you ready? I need 10-15
minutes."
K : “yes, I’m ready”

P : “Okay, I will do it now, please be cooperative”

(Then nurse examines the client's vital signs including blood pressure,
pulse, temperature, and breathing.)

P : "The result of the vital signs examination show the blood pressure is
90/60 mmhg, the temperature is 37.8, her breathing is 14x/minute and pulse
is 80x/minute”

I : “How about my daughter’s condition based on the examination? “

P : “The result of vital signs examination is normal, but her temperature is

higher than the normal state, which is 36.5-37.5

I : “What can you tell about the result? Will my child's condition get bettr?"

P : “I will inform these results to the doctor and ask for the recipe for
Galuh."

I : “Okay”

P : “I have done all the examination this morning. Please excuse me. if you
need any help, please call me or other nurses in the nurse station.”

I : “Okay, thank you”

P : “About 10-15 minutes after this, the nutritionist will come to the room to
observe and handle Galuh’s nutrition needs."

I : “Okay thank you for the information”

P: “Get well soon Galuh”

(15 minutes later, the nutritionist enters the jasmine room and observe
Galuh's nutrition needs)

G: “Good afternoon"
K: "Good afternoon"
G: "Let me introduce my self, my name is Devi Andriani from the
nutritionist. Is it true that the patient's name is Galuh?"
K: "Oh yeah right"

K: "So this is how it is, my aim mother came here to observe the fulfillment
of nutritional needs to adek galuh. Previously were you willing? Here I need
more or less 15-20 minutes.
K: "Fine, I'm willing"
G: "Mother, if I may know the weight of the first sister Galuh in the hospital,
how much mom?"
K: "38 kg nurse"
G: "Ooo means decreased, ma'am"
K: "Yes nurse, my son's appetite decreased nurse."
G: "Mother, please increase the dietary size of Galuh's sister. Because you
need adequate nutrition so that your fluid and nutritional needs can be
balanced."
K: "oh ... okay nurse"
G: "This is for Galuh’s lunch ma'am, this has been adjusted according to the
nutritional quantities that must be met."
K : "Is this the right portion? It's too small."
G: "Yes, this is right ma'am. The portion is served based on Galuh's nutrition
needs."
K : "My child is VVIP patient. I don't think that's the right portion. I cannot
accept this."
 G : "Do you want to change the meal for today's lunch, ma'am?"
K : "Absolutely. I want you to give her more food."
G : "Okay, I will take new meal for Galuh as requested."
(Then the nutritionist takes the new meal for Galuh)
G : “Here is the new meal for Galuh”
K : “This is the right portion. Thank you.”
G : “Please make sure Galuh eats her meal”
K : “ Alright"
G : “Is there something you want to ask, ma'am?”
K : “What are the food suggested for my daughter?"

M: "For sister galuh, you should start to multiply eating foods that are
nutritionally balanced, starting from rice to meet carbohydrate needs, fish for
protein needs, multiply to eat vegetables and fruit, and complete with milk
yes ma'am, and always drink water."
K: "Oh ... I see, nurse, then for next is there any taboo on it?"
M: "For the taboo, Galuh should avoids spicy food, and don't eat it at
random either at home or at school"
K: "Okay nurse"
M: "Then I will stay first mom, if you want to consult about nutrition, please
ask ma'am, I'm in the nutrition room"
K: "Okay nurse, thank you"
M: "You're welcome, ma'am."

(15 minutes later, there is visitor looks in Galuh's condition)

P* : “Hello, how are you? Is Galuh getting better?"
I : “Hello, no improvement so far”
P : Hello sweetie, how do you feel? Does it hurt?"”
K : “ Yes aunty, it's so painful"
 P* : “ I bring some chocolate and tasty food for yoy."
K : “yeay, thank you!"
P* : “Let me feed you"
I : “No, wait. She is not allowed to eat that kind of food yet."
P* : “Why?"
K : “Oh come on, mom"
I : “Don't you remember what the nutritionist told you earlier?"
K : "I am sorry aunty. It seems like I cannot eat these chocolate”
P* : "Come on, I only give her a little"
I : “No, please. I am afraid it will bring bad effects to her condition."
P* : “How can you be so disrespectful. I have brought the snacks all the way
here, then you refuse them?"
I : “I am terribly sorry"
P* : "God please. We've been friends for years but you forbid me to gige your
daughter her favorite food? What kind of hospital is this?
I : Please, I really apologize
P* : I am leaving
(After the visitor leaves the room. The mother feels so bad for refusing the
snacks she brought for Galuh)
