Transfer Professional License

Requirements:

• No Objection Certificate (NOC) from current facility

• Offer Letter from the hiring facility
• Malpractice Insurance from the hiring facility
• Log Book for the last 2 years (for surgical specialties)
• PSV report
• Valid passport copy
• Passport size photograph
• Medical fitness test (if applicant age was 65 and above)

• Physicians/Dentists : 3010AED
• Nurses/Allied Health/TCAM : 1010AED

Note:
1. No Objection Certificate must have the facility stamp and signature of Medical
Director or authorized person.
2. Offer letter must be issued recently (last 3 month), include the professional title,
facility stamp and signature of Medical Director or authorized person.
3. Transferring the full time DHA license will result in cancellation of all part time
license
4. Medical fitness test should be conducted only by medical fitness center- DHA
5. If facility name that applicant is applying for is not shown it means that the
facility need to add professional specialty and then applicant can transfer the
license

Submit Transfer Professional License Request

1. From Professional Home Page Click [Transfer Professional License] link

2. View your personal details > Click [Next]

3. Click [Add] to search for the facility

(a) Write a keyword in [Search Facility] text field > Click [Search] button to list
the result Or Click [Search] button (with empty key word) to list all available
facility > Click [Select] link next to the required facility > Click [Submit]

4. Click [Attach File] button to upload required document

a. Attach Malpractice Insurance, No objection Certificate, New Job offer

b. Click Attach File button next to *Medical fitness documents to attach

required document (if applicant age was 65 and above)

c. Click Attach File button next to *Upload latest passport size photograph
to attach required document

d. Click Attach File button next to *Upload valid passport copies (including
Visa copy) to attach required document

5. Click Attach File button next to *Upload PSV report to attach PSV report

6. Fill your UAE Address and mobile number for Zagel delivery, and Feed-in your
comments in Comments text area (if you have any) and then Click Submit button

7. Once you complete the process successfully a reference number will be generated
for your request

1. Go to your inbox

2. Form Pending Actions Click [Open] link to proceed your payment

3. Review your payment details > Click [Pay] button

4. Fill the payment page with the needed information
