Criteria Description
Physical Take a moment to reflect upon the things you are doing to take care of your
Self-Care: body in healthy ways:

1. How many hours of sleep am I getting in a day? Is it enough for


me?

2. Do I eat a balanced diet?

3. Do I watch my nutritional intake with every meal?

4. Do I keep myself active?

5. Do I regularly schedule and attend check-ups with my healthcare


providers? (Remember this includes more than just your doctor)

Emotional Take a moment to reflect upon the things you are doing to take care of your
Self-Care: feelings in healthy ways:

1. Do I maintain my personal and professional support systems?

2. Do I journal or talk about my feelings in healthy ways?

3. How well am I taking care of myself emotionally?

4. Do I participate in counselling or therapy as needed?

5. Do I take time to evaluate how my emotions have affected myself


and those around me?

Cognitive Take a moment to reflect upon the things you are doing to take care of your
Self-Care: mind and understand yourself better:

1. Do I practice mindfulness?

2. Are my thoughts and actions aligned?

3. Do I feel overwhelmed by specific areas of my life?

4. Do I engage in life-long learning?

5. Am I living in the real world or do I make my life a fantasy?


Social Take a moment to reflect upon the things you are doing to ensure you have
Self-Care: healthy relationships with others and the world around you:

1. Am I so caught up in technology that I have forgotten to interact


with real people whom I can see or touch?

2. Do I spend time with my family, friends and colleagues?

3. Do I take part in communities and activities that encourage positive


social connections?

4. How well do I take care of myself socially?

5. Amidst the current circumstances, do I connect with my friends and


family through phone or any other media?

Spiritual Take a moment to reflect upon the things you are doing to gain perspective
Self-Care: in your life:

1. What am I doing to keep my spirits lifted?

2. Do I regularly practice my own spiritual rituals? (e.g. prayer,


meditation)

3. How well do I take care of myself spiritually?

4. Do I have hope in what I do daily?

5. Do I spend time in nature?

Financial Take a moment to reflect upon the things you are doing to spend and save
Self-Care: responsibly:

1. Do I track my recent pay stubs?

2. Do I know my current spending habits?

3. Do I prioritize my debt payments?

4. Have I set an emergency fund target?

5. Do I invest toward my goals?
