DELIVERABLE REPORT
Background
Base on contract number #: 0936.0078, Date Issued: September 26th, 2019 with Project
ID # / FCO #: 100303.001.015.001.001 and Purpose: To support implementation of
ICT4D activities for FY 2018 Workplan, it was accomplishing a series work activity with
LINKAGES Indonesia staff (cross component) to develop ICT applications in support of
the above tasks in the LINKAGES FY18 work plan. This modification contract is to adjust
ADDITIONAL SCOPE OF WORK (SOW) & DELIVERABLE for new software/application
development and additional deliverable that not defined in the previous contract.
In this new SOW and Deliverable, the consultant will work with LINKAGES Indonesia
staff (cross component) to further develop ICT applications in support of the above tasks
in the LINKAGES FY18 work plan. He shall work under the overall supervision of the
ICT4D Advisor.
B. Activities:
1. E-Mentoring/Mentorship Web Base System (maximum40 days) :
a. Modify the existing e-mentoring application with reference to the
requirements agreed upon by Provincial Health Office, district health
office and or End user (Health Facilities and CSO) based on selected
area provided by LINKAGES team
b. Coding, testing, refining (5 iteration) for each modules/menus/feature that
have been developed
c. Present the Final version of the application, focusing on the new key
features, to LINKAGES team for approval.
d. Conduct quality and performance tests and fixes with ICT technical
advisor.
e. Migrate and create table database to integrated ODK platform database
into web base data and visualize the data into graph and or chart.
f. Final reporting and lesson learn document.
C. Expected Outputs:
1. Upgrading E-mentoring/mentorship and it has data interoperability among
Jak-track and SIMONA
2. Additional Menus/function for “Partner Notification” Compilating and reporting
system” in the Jak-track application (for Clinic/Puskesmas) and in the
SIMONA web base system (for DHO and PHO)
3. Keep maintaining Jak-track, SIMONA and all relevant application running
well.
This is a Monthly Progress Report for accomplish reporting for several deliverables as
agreed in TOR/SOW for period 01 August to 31 August 2018. This monthly report is
expanding several activity and deliverable that have been reporting in last monthly report.
Sub deliverable/Activities:
a) Upgrade and/or expand application/software “Jak-track” app system
In this reporting period, mainly work on modify the DOKLING application for additional
function of Facility to Facility transfer with reference to the requirements agreed upon by
Provincial Health Office, district health office and or End user (Health Facilities and CSO)
based on selected area provided by LINKAGES team. New module was conducted
Coding, testing with ICT4D advisor and CSU Advisor of LINKAGES Project, refining for
Facility to Facility transfer modules/menus/feature that have been developed.
There are 3 different flow, logic and view has been integrating into Dashboard and one
“main Menu” that it is called “PDP/Enroll” Menu. The flow it is following:
1. Update module user management at timesheet application/tools
Before update this form have field account level, units , etc. after update this
page/module just insert/create basic account info, after account created use role can
manage at unit management page, this required to hirarki dashboar info and report to
superadmin, admin and employee
2. Backup source code and database at server 103.6.53.122, create trigger ODK
Collect synchrony with timesheet web application
All source and database at server 103.6.53.122 was backup to github.com with
private repository can be pull/cloning if needed
3. Develop button export chart at report page timesheet web base application
All chart can be print and export to file with format, .PNG, .JPEG, .PDF, .SVG
(vector) with click button at top right on the portlet chart.
4. Develop filter form chart at report page timesheet web base application
With this filter form, superadmin and admin can be filter data to vizualition graphic
report with base data on units, all unit or one person staff.
5. Update and fixing issue at form planned task & unplanned task timesheet project web
base application
Before update this form just have field description and status, after upgrade this form
have field completed, as like type of task, detail type, platform used, etc
6. Develop module report graphic analytics to view visualization task all staff
