Scope & Viability
This project involves the development of an online system for tracking patients’ medical history
across all different practitioners, clinics and hospitals i.e. a centralized interconnected system
for storing patient records. The system will enable any medical practitioner who is registered to
view and add to a patient’s medical record. The patient therefore, will be able to visit and
practitioner or clinic and their records will be available, also being able to view their medical
history and print a report if they wish to do so.

For the proposed project to be successful and benefit the health care organizations, the Care
Clinic system should be able to achieve client requirements and desires at best. This online
system viability is judged upon its ability to be profitable and whether adopting this system will
prove beneficial in terms of time and cost for both the development team and the clients.

Factors taken into consideration to determine Care Clinic’s viability include:


 Cost – the project has to be within the allocated budget. The resources to design and
develop the system should be carefully considered to avoid exceeding the designated
budget.
 Time – allocated tasks should be achieved before deadline. Client requirements should be
completed within the specific timeframes to avoid delays.
 Manpower – losing key staff members who are involved in developing the project can have
the Client Care system lose its viability. If a potential member is lost, the system might have
delayed development unless the organization recruits another suitable individual.
 Quality- the quality of the system can also affect its viability. Resources that are most
suitable should be used, keeping in mind the budget and availability of the materials.

Executive summary
The implementation of any system requirement, whether it be an upgrade of the current
system or the development of a new one, a thorough research and analysis is essential. The
need along with the influence that the system may have must be well calculated. Hence, the
project feasibility report is written for the project of a health care system named “Care Clinic”,
developed by Team Acell of the University of the South Pacific.

Families having family doctors are becoming quite common. However, there are various
occasions where families might be visiting different clinics, hospitals, and doctors when family
doctors are not available. Even in such cases, patients can still receive treatment from multiple
medical practitioners. Due to the hassle of keeping track of the patient’s medical history,
allowing various medical practitioners to view and update patient records, and providing
prescriptions and medications, the Government of the day passed a legislation to design an
online system that efficiently preserves patient’s medical information at a single synchronized
storage.
In most cases, patient’s medical record may be available at more than one clinic or hospital,
done by various medical practitioners which can prove hectic to keep a single patient’s records
at different places and not being able to access these records when needed. This can threaten
the patient’s health as full or suitable treatment for illnesses are not given due to not knowing
about a patient fully.

Before starting the development of this project, a feasibility study was conducted that
concentrated on the proposed systems desired needs. The feasibility report highlights whether
the Client Care system should be taken into consideration for development and
implementation. Major findings pertaining the online patient record keeping system was done
in respective of its economical, technical, risk, schedule and time, resource, and social/legal
feasibility.

This report will highlight on the results of the undertaken feasibility study along with
alternatives that can be used to the health care system. Information stated in this report is
collected and composed through cooperative effort of Team Acell along with the collaborative
assistance gained from medical practitioners. Satisfactory information from researches
regarding the need for a health care system that can keep track of patient’s medical history
online was procured to aid the development team in successfully completing the feasibility
report.

Background Information
The Republic of the Fiji laying in the South Pacific Ocean, part of Oceania, is an island country in
Melanesia. It consists of more than 330 islands with the total area mass of 18,300 square
kilometers. Fiji is one of the most developed countries in the South Pacific, however it is still
developing. Many sectors, in our case the health sector, has a lot of room to advance. Fiji’s
health care system may approach adequate standard, however in the ever so developing
worlds, it can become very basic or even inefficient.

By international standards, the current system in use are isolated. There are a number of
hospitals, health centers, nursing stations, as well as private health clinics and these are
geographically diverse. The primary goal of the health care is to provide convenient, efficient,
and effective medical consultation and treatment for the patients. It also aims to make it easier
for the doctors by making a working environment which has up-to-date equipment and data so
that it is easier to diagnose/treat any and all patients coming for a checkup or treatment. A
concern arises as these health care providers are not linked and do not have a way to share
patient information. A patient who has visited a clinic for treatment has a record at that clinic
but if the same patient visits a different, the information from the previous clinic would not be
available. This is a problem as initial information regarding treatment and medication, medical
conditions, and other relevant information are not known by the new medical practitioner, at
the very least, it is an inconvenience to both the patient and the provider.

Elicitation Technique Applied
To gather requirements in regards to the Care Clinic online system, three types of elicitation
techniques were adopted:

Interview – This was the primary method to collect information about the current system and
the need for a new system since the team were able to communicate face-to-face with the
relevant stakeholders. Interviews aided in obtaining detailed information about the opinions
and viewpoints of stakeholders in a highly interactive manner, building a faster assessment
process.

Sampling Hard Data – Documentation about current system was viewed and analyzed to better
understand how it operated and the processes and procedures that are followed. This provided
a guide to implement the new system in accordance to the policies that the current system has
in place with regard to its access, management, handling, security and other issues.

Observation – The true understanding of how the current system worked was through
observing the workers on how they run the daily activities. This is useful to find process or
methods of doing work that have been created by workers which may differ from official
procedures outlined in policies and documents.

Proposed System
Description of the System
The Care Clinic online system is a project being designed to aid doctors and other medical
practitioners in keeping records of a patient’s medical history. This system can come on hand
when patient’s, whose family doctors are not available, visit other clinics, hospitals, and/or see
other doctors. The system would allow the patient to get treatment from multiple doctors with
the doctors having permission to view and update patient record. However, to allow
authorization for the manipulation of patient records, the doctor has to be registered to use the
online system.

Moreover, to update a patient’s medical history, the patient itself has to be registered in the
system, where after the completion of the treatment and/or receiving medication or
prescription from a medical practitioner, the doctor may login into the online system, search,
and update the patients record. Non- registered patient and doctors may be asked to register
themselves first by filling out the online registration forms through the input of various details
such as name, ID, contact details, and emails to have access to the system.

Care Clinic online system shall allow customers to login into the system themselves in order to
view their medical history and print a report should there be any need to. Additionally, the
system shall allow patients to view their medical history through the use of their authorized
smartphones. Information stored on Care Clinic can be shared amongst hospitals/ clinics

Advantage & Disadvantage of the Care Clinic online System


Advantages
 Data availability: data is available around the clock
 Data access: data can be accessed from anywhere
 Security and data confidentiality: data is kept secured since the system will need
authentic authorization to access patient records such as Patient or doctor’s ID and
username.
 Data integrity and consistency: data integrity is maintained and redundancy is reduced.
This ensures that there is no conflict amongst existing and updated records about the
patient.
 Storage: data about the patient is efficiently, effectively, and easily stored. Physical
storage is replaced with cloud storage, minimizing storage space as required by
traditional methods of storing data in file.
 Data backup: data and information about the patients are backed up to significantly
minimize the risk of these data and information being lost in natural or man-made
disasters.
 Data sharing: data can be shared amongst various department, clinic, hospitals, and/or
doctors to provide correct diagnosis and patient treatment.
 Easy to use: the system is in natural language and easy to understand and use.

Disadvantages

Staffing
The Care Clinic will entirely be a new system that will be implemented for the use of the
medical practitioners as well as the patients. It will be installed at clinics and/or hospitals, as
well at the site of patients, hence, it is imperative to have the right technical support and the
right team to install this online system. To use the Care Clinic to its full potential, the users must
be fully trained to understand how the system operates or be versatile enough to use the
system without any issues. The most important people that may be influenced by this system
includes the medical practitioners, patients, and the IT administrators.

Basic Layout (Architecture)

Figure 2.0 shows the basic view of the network architecture of the Care Clinic online system. Hospitals/clinics and/or patient
devices act as clients that are connected to a server residing locally in their region. These local severs are connected to each
other via the internet. Clients request for information about the patients from their local servers. If patient information is
unavailable at their local server, these servers communicate with other servers through the internet to check and retrieve the
requested information from one another. Information added or updated about a client in synchronized to other servers to
maintain integrity

Cost Benefit Analysis


Cost benefit analysis for Care Clinic provides the assessment of costs associated with the
implementation of the online system and its benefit to the health care organization, medical
practitioners, or the society. The implementation of Care Clinic system is part of the health care
economy, hence the study of economic factors that would influence the decision about clinical
treatment of patients, specialists, insurance agencies, and public health care agencies should be
carefully analyzed.

Care Clinic online system would decrease the expenses through the advantages it will have over
the present system of the health care organizations. This includes decline in labor and staffing
hours from physically entering existing and non-existing patient records. Additionally, less time
would be required for searching, examining, and updating patient’s medical history. The time
saved on gathering records from other departments, clinics or hospital, as well as saving up on

physical resources for manually storing patient data are extra rates of profitability from the
implementation of the proposed system.

Cost Savings:
Assuming 2 minutes to fetch a patient record manually. Half a minute to fetch patient record
using Care Clinic online system. 150 patients per major hospital. And, $5 per hour wage of
receptionist.
2 min x 150 patients = 300 minutes = 5 hours
0.5 min x 150 patients = 75 minutes = 1.25 hours
(5hrs-1.25hrs) x $5 = $18.75 savings per day
$18.75 x 6 days x 52 weeks = $5850 savings per day per major hospital
$5850 x 21 major hospitals = $122850 savings per year

Computer Software Requirements


The Care Clinic online system is a sophisticated software program that would make the work of
health care organizations, as well as the medical practitioners and patients work efficient.
However, to achieve that, the proposed system has to keep up with the requirements by being
able to handle speed, process, and communication within and across the system.

Therefore, Care Clinic system should be integrated with software that would provide the users
with a robust system. Recommended software requirements to par with users preferences and
allow the system to perform at its best may include:
 The software application itself.
 Hosting – managed locally.
 Web browser – Chrome (Version: 70.0.3538.77 and above) or Firefox (Version: 62.0
and above)
 Adobe Flash Player

 Operating system – Windows 8 or above

Site Possibilities
Care Clinic system will be hosted online and given access permission to health care
organizations such as public and private hospitals, as well as nursing stations. The care clinic
system will involve both the urban and rural regions.

This patient record keeping system will also be accessed to registered patients who would like
to view and print their medical history if/when needed. Costing at sites may differ especially in
the rural regions and nursing stations that may need some resources provided such as laptops,
servers, and/or staff training.

A region will have servers located at the major hospitals and the smaller hospitals, clinics, staffs,
and patients will request data stored at that local server. If data is not found at the local server,
severs can then connect itself to other servers via internet to retrieve the requested
information. The proposed system can have a central database or a central location for housing
the necessary infrastructure. However, a more suitable approach would be to have a
distributed system where the servers are located at different locations, serving mainly those
areas. This would contribute towards data traffic and barges and the system would perform
faster.

Care Clinic is an online patient record keeping system, hence the most important consideration
regarding this project is the permission for data and information access and retrieval pertaining
different parties, that is, the medical practitioners, patients, and other relevant stakeholders.
An essential requirement would be to separate these parties from one another so that each will
only have access to what is necessary.

One of the commonly used technique for determining a new project’s efficiency prior to its
usage, is doing an economic feasibility. For the implementation of the Care Clinic system, it is
vital to understand the financial cases, or how the system would be implemented accurately
and within workable budget

The application software to be developed is priced at $225000. Licensing for Windows 10


operating system will cost around $200 per user per year. The Samsung Galaxy Tab A 8.0 has
been selected as it will meet system requirements and is reasonably priced at $379. Cat 6
Ethernet cables will be used to connect computers in local networks. Dells PowerEdge R7425
rack mounted server has been chosen due to it high upgradability and built in data analytics
tools provided ($8405). Along with the servers, Huawei AR121 AR120 Series router with a built
in switch has been chosen ($550.77).

Cost of maintenance is calculated at 19% total hardware cost for the first year, 21% for the
second year, and 22% for the third and capped at 25% from the fourth year. This includes costs
such as replacement of devices, cost of upkeep, upgrades etc.

Initial cost of training will be large as all members will need to be trained. After the initial
training, lesser numbers of people will need training and this number has been kept as a
constant 20 people as an approximation. The total cost of implementation and running for the
1st year comes to $413,948.

Technical Feasibility
Hardware
Technical feasibility of the proposed system was conducted to evaluate factors about the
hardware and the software resources. Rural nursing stations do not have necessary resources
to run PC’s that have to be connected to a power source as they may not have access to
electricity. Staff at these stations are required to make rounds around the communities for
checkups. This will require provision of mobile devices such as tablets.

In other health centers and private clinics, existing PC’s would suffice if they meet the
following minimum requirements:
- Intel core i3 processor (or equivalent)
- 2GB RAM

- 1GB free disk space

Accessing the Care Clinic system will require an internet connection.

Software
For successful operation of the Care Clinic system, the required software includes an up-to-
date web browser, adobe flash player and windows 8 and above operating system. Current
health care provider’s systems may need to be updated in order to implement the Care Clinic
system.

Skills
In order to build the proposed system, the development team will require the following skills:
- Programming knowledge: C, JAVA, Python, HTML
- Database Creation and Optimization: SQL
- Networking
- Web development
- Software testing
- Design and analysis
- Risk management
- Scheduling and project coordination

Resource and Time Feasibility


Resource and time feasibility study highlights the number of resources that would be required
for the Care Clinic system as well as any other dependent features through optimal
employment of available resources. This study would also take into account whether the
proposed system is interrupting current business activity (if any).

The designing team of the Care Clinic system have kept in mind about the resources that are
already available, as well as the resources that might be needed by the organization who
intend to use our online patient record keeping system. Time is considered as the ultimate
means of gaining profit, therefore, the Care Clinic system can be easily adapted by the health
care providers along with the staff to be used in the most efficient way.

Resources for the Care Clinic system that may be required include:

The above mentioned resources are available locally. Resource costs are broken down in
economic feasibility (figure 4.0)

The Client Care system uses timeline as a milestone to compare outline the system
development and implementation. The development of the Care Clinic system, its
installation, and staff training are shown in the Gantt chart below (figure 5.0).
Approximated time needed for the Care Clinic system to be complete would be 4.5
months.

Risk Feasibility
SWOT Analysis

To identify the weaknesses and strengths of the Care Clinic project, SWOT analysis
technique was used. This technique also worked on identifying the opportunities and
threats in relation to the online system created.

The importance of the SWOT analysis technique for the Care Clinic online system is to
ensure that the health care organizations are well informed about the record keeping
system’s services that it would administer to the organization and the targeted audience
who will use this system.

Since the government has legislated the Care Clinic system, there would not be any legal issues
pertaining to its development.
Also, since all medical practitioners have agreed to be a part of this system, there will not be
any social issues on their part. However, some patients may not be comfortable with having
their information being accessible to all medical professionals. This raises concerns of privacy as
well as stealing of information (such as selling patient data to insurance companies). Therefore,
controls must be in place to give access to only those practitioners whom the patients agree to
have their information shared with. In cases of emergency, trusted privileged practitioners
should be able to access patient information or give access to specific practitioners for the
particular case.

Final Recommendation
The main functionality of the Care Clinic system is to provide means of online tracking system
for patient’s medical history. When compared to the traditional methods of patient record
keeping, the client care system provides a more efficient service. This project works towards

minimizing cost, reducing labor time, processing information, maintaining integrity, and
retrieving information about a patient at a much faster pace.

By implementing the Care Clinic system, health organizations will also move in parallel to the
ever advancing technologies which is vital, especially when it comes to providing better health
services to the society. The system also focuses on providing efficient data maintenance,
integrity, and consistency.

With the ever growing medical concerns and the rate of people getting ill being on a rise, it is a
necessity for hospitals to have a patient’s medical history and medical records to make medical
diagnosis easier. Doctors having a patient’s previous medical records eliminates the need for
asking the patient for their medical history, which can be incorrect or not told in enough detail,
and the need to ask the patient for any allergies and other medical conditions the patient may
have. Collaboration between different practitioners and easy access to patient information will
be highly valuable and beneficial to everyone in the current pandemic of Covid-19.
All of these hassles can be overcome with the implementation of the Care Clinic system, which
saves time and effectively and efficiently provides the doctors and nurses the information they
would require about a patient at the press of a button.
