AFFIDAVIT OF SELF-ADJUDICATION WITH DEED OF ABSOLUTE SALE

I, FLORINA S. COCOY (VENDOR), of legal age, single, Filipino citizen, with


residence and postal address at Poblacion, Atok, Benguet, Philippines after having been
duly sworn to in accordance with the law hereby depose and states:

1. That I am the only surviving sister of the late ELPEDIO S. COCOY (Sometimes his
name is spelled as ELPIDIO) who died intestate on October 25, 2004 at Poblacion,
Atok, Benguet (copy of his death is hereto attached as Annex “A” and made an
integral part hereof), who left a certain parcel of land covered and embraced by
ASSESSMENT OF REAL PROPERTY (ARP) No. 99-001-0276, with an area of
ELEVEN THOUSAND SIX HUNDRED (11,600) SQUARE METERS, more or less,
with ASSESSED VALUE OF SEVEN THOUSAND TWO HUNDRED AND THIRTY
PESOS (Php 7,230.00) and ADJUSTED MARKET VALUE of EIGHTEEN
THOUSAND AND EIGHTY PESOS (Php 18,080.00), and more particularly described
as follows:
ASSESSMENT OF REAL PROPERTY (ARP) No. 99-001-0276
North: Lots 008 & 009 South: Section 57
East: Lots 019 & 021 Section 57 & 58
(For complete technical description of a Copy of ARP No. 99-001-02769 is hereto
attached and marked as Annex “B”)
2. That at the time of his death, said decedent left no will and obligation;
3. That it is to the best of my interest I hereto adjudicate unto myself the above-
mentioned parcel of land;

BY WAY OF DEED OF ABSOLUTE SALE

4. That for this consideration of the total sum of TWENTY-FIVE THOUSAND PESOS
(Php 25,000.00), Philippine Currency, receipt of which is hereby acknowledged by
GREGORIO J. LUIS (VENDEE), of legal age, single, Filipino citizen, and a resident
of Poblacion, Atok, Benguet, I hereby SELL, TRANSFER, and CONVEY, in a
manner absolute and irrevocable, unto the hereunder named GREGORIO J. LUIS,
his heirs and assigns, the above-mentioned parcel free from any liens and
encumbrances;
5. That I hereby warrant ownership as evidenced by the valid Assessment of Real
Property to and peaceful possession of the above-described parcel of land herein
sold and conveyed unto GREGORIO J. LUIS and further warrant that said Tax
Declaration of Real Property is free from all liens and encumbrances whatsoever;
6. That the above-described property not having been registered under the Spanish
Mortgage Law nor under Act 496, I and Gregorio J. Luis agreed to register this
instrument under the Revised Administrative Act No. 3344;
7. That the above- described property is not tenanted nor devoted to the production of
rice and/or corn in whole or in part as of October 21, 1972 and/ or on the date of
registration hereof;
8. That this instrument is published in a local newspaper of general circulation for three
(3) consecutive weeks in compliance with its proper and due registration;
9. I am executing this affidavit to attest to the veracity and truthfulness of the foregoing
statements and for all legal intents and purposes it may serve.

ACKNOWLEDGEMENT
Before me a notary public for and in the City of Baguio, this 3 rd day of December,
2012, personally came and appeared the above-named parties and presented to me their
CTC and BIR TIN Card respectively, an integrally complete instrument for identification and
she acknowledged to me that the same is her own free act and deed.
I further certify that the foregoing instrument, consisting of two (2) pages, including
the acknowledgement relate to AFFIDAVIT OF SELF-ADJUDICATION WITH DEED OF
ABSOLUTE SALE of one parcel of land, signed by the affiant/vendor thereto and her
witnesses, including the Vendee;
