Assessment Portfolio

Annotated List
1. Essay Response
The prompt for this response would be given to the students at the end of class both
physically and digitally. Students would be given verbal directions for the guidelines on
specific instruction and how the document should be formatted. Students in their own
time within the allotted time would complete the assignment and turn it in to the teacher.
The students will submit one rough draft and one final paper. The teacher will provide
feedback for the rough draft on how the student can improve their responses. The final
submission will be graded by the teacher based on completion, content, grammar,
spelling, and flow.

2. Self-Assessment Worksheet
The worksheet would be passed out preferably before the beginning of class started so the
teacher did not have to use time passing out the worksheet during class. Students would
be given verbal directions on how to fill out the worksheet as well as having the
directions printed on the sheet itself. Students will fill out the worksheet on their own and
would be expected to be quiet while doing so. The students would turn in their worksheet
to the teacher once they have completed them. This could be utilized in many situations,
but one example would be to gather student feedback on what we are currently working
on in class and using their answers to guide future lesson plans and activities. These are
student and opinion based assessment, so that should be kept in mind when considering
the reliability and validity of the assessment. The teacher would read each response and
take note to the common trends of responses. As the year progresses the questions could
be revised and used as a tool of reflection and growth if given to the students again. After
analyzing, the documents should be filed away or saved for future documentation and
record of progress.

3. Playing Test
This assignment will be given to the students at the beginning of the week and as the
week progresses we would work on the selection as an ensemble. The students would
also have time to practice their test on their own throughout the week. The students
would each play their test one at a time in front of the class. This would allow for the
teacher to see progression in the technical aspect of what is assigned as well as get the
students used to performing in front of others. The teacher would grade this assignment
based on tone, note accuracy, and rhythmic accuracy.

4. Recording Assignment
Recording assignments will be assigned to the students in class. Students will have to
record a selection from a piece, warmup, and/or etude that is being worked on in class to

evaluate accuracy or other elements that are being observed. Students will be given a set
amount of time to complete the recording and submit it to the teacher via email.
Accommodations will be made if needed. The teacher will assess each student
individually and take notes on what the common themes are in regards to the tests and
then give a completion grade. This will allow the teacher to see common errors and
misunderstandings that the students are having with the selection. This will also allow the
teacher to see what is going well. This will help the teacher guide future lesson plans and
help them determine strategies on how to address issues and formulate solutions.
Recording assignments can gauge individual growth and accuracy. This also allows
students to record their assignment as many times as they would like to get the result they
want. It also alleviates performance anxiety and stress that may be caused when having to
play in front of the entire ensemble.

5. Written Test
The written test would be administered individually in a classroom setting. The date of
this test will be given in advance and a review of what material will be on the test will
happen a week prior. The teacher will remind the students that the test is individual and
to follow their directions given and to do their best. They will have the time allotted to
take the test and once they are done and have turned the test into the teacher, they can
quietly work on other assignments they may have. There will be no notes or technology
used to take the exam. The teacher will then grade the tests and record the results. The
students will receive their graded tests back as soon as possible and once they do, the
teacher will go over the correct answers and will answer any questions that the students
might have. The teacher will then recollect the graded exams for use in the portfolios.
The test will be edited over time as needed for clarity and to remain relevant for what
material is learned in class.

6. Bell Work/Exit Ticket


Students will be given a piece of paper at the beginning of the week with slots for every
day of the week. Each day of the week there will be something on the board in a
predetermined spot that the students will be aware of that will have a question, fill in the
blank, rhythm, or something of importance that the students will have to answer on the
piece of paper before class starts or before they leave. Each question will only be
available for that day. At the end of the week the students will have to present their
finished sheet to the teacher as an Exit Ticket. The teacher will then grade the sheet,
record the data, and review it with the students the following Monday. The students will
give the graded assignment back and the teacher will file it in the portfolios. This will
assess knowledge and accuracy on whatever questions are given, which could be a
variety of subjects.

7. Project
Projects would be described in full detail during class and students will receive a copy of
instructions both physically and digitally. Students would be given example of each
prompt they could complete and if they would like to branch out of what was given, they
could do something that is not described in the instructions by meeting with the teacher
one-on-one for permission. This will be a long term project, so the teacher will outline

the timeline of the project. The teacher will also give class time for students to utilized
resources within the school and classroom, as well as ask questions if needed. Students
will submit their project to teacher as well as present it to the class. The teacher will
assess the project based on how well the student followed the directions and guidelines
given. The instructions and guidelines will be adjusted based on what went well and what
did not.

8. Portfolio
The contents of the portfolio will be collected over time and organized by the teacher. It
will be a way to see the progress the students are making within the program over time. It
is also a way to ensure that progress is being made from August to December or August
to May. This could also be a way to show a student’s progress that has been with your
program their entirety of being in high school. All of the assignments the student has
completed throughout their time in the program will be kept chronologically within their
file.

7. Project

As the semester progresses, keep track of new skills and terms you learn. At the end of

the semester you will use this to complete a project reflecting those skills and terms that
you have acquired.

You will decide how you will reflect this knowledge. This could be done multiple ways:
composition using notation software, composition using garage band, essay, arrangement,
etc.

Each project will include a compiled list of skills and terms that the student has learned
and a rational as to why and how the chosen project will reflect the list given. The project
will then be approved by the teacher. The student will submit 2 rough drafts, a final
project, and present the project to the class. The dates at which those will occur will be
given at a later time.

8. Portfolio

This will be a digital and/or physical compilation of each student’s assignments, tests,
and projects completed throughout the semester. If a student is in the ensemble for
multiple semesters, a new file is made each semester, but all information is kept and
organized per student. This is able to show the students’ progress throughout their time in
the program and through the semester.
