1. INCIDENTS:
1. At 1540 hrs, in A-100 (south side), near Descon Tool Shop, refractory shi ing from truck was in progress while no road closure permit
and no cold work permit was issued for the job..
2. Between A-300 and A-400, a crane was parked for catalyst loading ac vity and its out-riggers covered more than half road. Another 50
Ton crane (Qatar#211639) crossed while access was not enough to cross from there, during crossing, crane half tyres move over from
parked crane out riggers pad. Crane operator was Mr. Asim (E.no 3330) and Rigging supervisor was Mushtaq (E.no 9042).
3. In A-500 (810-V11), inside stripper column, 220VAC light was being used without armored cable / PVC pipe sleeving while earthing of
metallic structure was also missing.
4. 4. At 0210 hrs, in A-300, Hydro je ng job was in progress, only cold work permit was issued for the job while hot work permit was also
required.

2. GOOD OBSERVATIONS: 5. UNSAFE CONDITIONS:

— 1. At 1230Hrs, in A-1010 (1010-V6), Cu ng grinding job
of lines (20GVA 33B3/4'') at 1010-V6 was in progress

4. UNSAFE ACTS:
1. In U li es, at 940-V53 (Decarbonator water Tank), Elec-
trical genset used for power supply was not grounded.
2. In U li es, at 940-V3(L.C Flash drum), on second
pla orm of 940-V3 ladder used on scaffolding was cou-
pled with metallic wire from one side instead of coupler.
3. At 1220 hrs, in A-1010 (1010-P4B), welding machine
(genset) was being used without earthing.
4. At 0155 hrs, in U li es (940-B1A), inside 940-B1A
(Furnace sec on), cleaning job was in progress while
SBM Mr. Wasiq Was without Walkie-Talkie. Moreover,
same observa on was also observed in A-300 at 284-
H50B (Thermal Heater)
5. At 2045 hrs, in A-200 (411-V4), one worker Mr. Akbar
was performing cleaning job of vessel trays near 411-V4.
His hands were contaminated with oil as he was wearing
co on gloves
6. At 0130 hrs, in A-100 (110-H1), three workers perform-
ing cleaning job inside 110-H1 (Heater) were not wearing
their dust mask as condi ons inside the heater were
dusty.
7. In A-300 (284– E13), 2 Ton sling was being used for rig-
ging ac vity of Exchanger tube bundle. The sling
a ached with chain block was found damaged.
8. At 0010 hrs, in A-200 (411-V7), during RT ac vity the two
SBMs deputed by RT coordinator Mr. Umer, le their
posi on before comple ng the ac vity, there were no
SBM on two sides of barrica on.
9. At 0830 hrs., in A-200 ba ery limit side, crane (JS-5405)
was moving without banks-man.
10. At 0100 hrs, in A-300 (284-H50), openings inside heater
fire box due to removal of burners were not completely
covered and can be lted that has the poten al of falling
hazard.
11. At 0200 hrs, in A-300 (284-H50), a transformer for
ligh ng in confined space was present on floor near 284-
V25, while water from Hydro je ng of AFC made the
transformer wet.
12. At 0130hrs, in A-400 (300-V1), Grinder work was in pro-
gress, sparks were coming out of canopy.
13. In A-300 (U lity Point 300-UP-20&06), PWHT machine
was in running condi on while fire ex nguisher was not
present there.
14. In A-100 (110-V1), manholes at 2nd, 3rd and 4th
pla orms were not barricaded while no entry was being
done through them a er end of day shi .
15. At 0920 hrs., in A-100 (100-V9), during li ing of pla orm,
worker movement was observed in the cordon off area.
