Mentor Evaluation Form

Dear Mentor,
You have been chosen by this student to evaluate certain physical aspects of his or her Senior Project. It is
necessary for the student to seek an evaluation because most of the physical part of the Senior Project is
completed outside of class. Your observations are extremely important to this student’s Senior Project teacher
because your responses assist in the overall evaluation of this student’s physical project. Thank you for taking
the time to complete this evaluation.

1. Describe the project:


Kelley has attended many boxing classes that have improved her physical abilities within the class. We are able
to assess this based on her impact score at the end of class, where we can evaluate where she can improve for
the next class.

2. Based on the final project and this student’s skill level, how many hours can you estimate that the
student spent on this project?
About 15 hours.

3. What impressed you the most about this project?


I was impressed by the challenge that this project presents to young adults in trying something new, and
the skills that are reinforced in doing so. I appreciate and am impressed anytime anyone chooses to learn
something as daunting as boxing may seem, but sticking with it builds confidence and technique and character
in young adults. Kelley has done a fantastic job with all of it.

4. What do you think could have been improved?


I didn't really see too many flaws at all. With the project or Kelley’s participation in it.

5. Where did you think that this student showed the most growth?
Kelley definitely improved her all-around skills. Her punching technique, footwork, guard and head
movement have all made large strides of improvement. Kelley was already physically fit enough to handle the
workout, but her work output, cardio, and power have all steadily improved, as well.

6. Discuss the student’s professionalism.


Kelley’s professionalism has been substantial since she has joined us at TBC. Kelley is a very
professional student and has been a pleasure to have in the gym every week.
