MEMORANDUM

SUBJECT : Amended Guidelines on the Protocol in the Issuance of Travel

Authority

1. References:
a. Memorandum from TDO dated May 14, 2020 with the same subject as
above; and

b. Memorandum from this Office dated May 10, 2020 with subject: Protocol
in the Issuance of Travel Authority to Locally Stranded Individuals (LSIs)
and Returning Overseas Filipinos (ROFs).

2. This pertains to the issuance of Travel Authority to LSIs and ROFs.

3. In this regard, the following are the amended guidelines to be undertaken in

the granting of Travel Authority by the Local Task Force CV Shield:

Issuing Authority Criteria

LSI/ROF from a barangay (origin) moving to another
Chief of Police
barangay (destination) belonging to the same municipality.
LSI/ROF from a municipality (origin) moving to another
Provincial Director municipality/city (destination) belonging to the same
province.
LSI/ROF from a province (origin) moving to another
province (destination).
Regional Director
LSI/ROF from a region (origin) moving to another region
(destination).

4. Requirements for the issuance of travel authority:

a. Medical Clearance Certificate issued by the City/Municipal Health Office

based on the following conditions for LSI:
1) That the LSIs is neither a contact, suspect or probable or a confirmed
COVID-19 case;

 2) That the LSI completed a 14-day quarantine based on the quarantine
standards set by the DOH; or
3) That LSI confirmed as a COVID-19 case was tested negative RT-
PCR-twice.
b. Certification from the Bureau of Quarantine (BOQ) for ROFs and COVID-
19 negative RT-PCR results;
c. List of LSIs/ROFs;
d. Point of origin and their destination;
e. Travel date; and
f. Name of drivers, vehicle to be used with plate numbers.

5. Requirement on the Certificate of Acceptance by the receiving LGU is no

longer required. However, Offices/Units issuing the travel authority shall also forward
a copy of the travel authority and list of LSIs/ROFs to Offices/Units who has jurisdiction
on the final destination of LSIs/ROFs to serve as ready reference in case of contact
tracing.
