medications with proper dosage
appropriate for their age and weight.
Hypertension has a high preventive
potential, if the client eats healthy food
and vitamins every day, it can be easily
prevented because the client will have a
much stronger immune system. Mrs.
Cristina Roncesvalles llooks like to be
worried about the health deficit they have.
Because of this, they want this deficit to
be gone immediately. Even their family
members especially their children are
worried and gives full attention to his wife
that has hypertension.

Alcohol drinking as 4.67 2 Absolutely it is a health threat because

a health threat alcohol has an adverse effect on the
person’s body such as liver, pancreas that
can cause cancer. It can be modified easily
because the person can change their
hobbies and vices in a good way instead of
alcohol drinking. When it comes to
 preventive potential if health teaching
about alcoholism is performed and if they
will read to get an info about being
alcoholic for the betterment of their lives.
They considered this threat as a problem
needing urgent attention so that they are
only drink alcohol occasionally.

Cigarette smoking 4.17 3 Cigarette smoking is absolutely a health

as a health threat threat because it has an adverse effect on
the respiratory of the person and more. It
can be modified easily and cured when the
client stopped this vice and given correctly
medication to overcome this threat. It has
a high preventive potential if right
interventions with rationale and health
teachings are done to the family. The
client considered this as a problem but he
is still doing this vice in his everyday life
and can take at least 1 pack of cigarette in
one day.

Insufficient hours of 4.17 3 It is a health threat because if you don’t

sleep as a health get enough sleep, it can cause high blood
pressure, diabetes, heart attack, etc. It can

be easily modifiable if Mrs. Roncesvalles
know how to balance his hours of sleeping
even though he has a farming at the
morning. This threat has a high preventive
potential if proper nursing interventions
and health teaching are performed in the
family especially Mrs. Roncesvalles. SHe
did not totally consider this threat as a
problem because it is his job that need to
become diligent and hard worker to help
his family in home and other expenses

Stress provoking 4.17 3 It is a health threat because interpersonal

situations as conflicts can affect the relationship of the
family with each other. It can be easily
manifested by
modifiable if the family knows how to
financial problem as properly manage and handle this kind of
a health threat problem. This can be done through
changing behavior of the family by
sharing their problem to each other. This
threat has a high preventive potential if the
 family has the knowledge to fix
interpersonal conflicts. According to Mrs.
Roncesvalles they do not totally perceive
this as a problem because their children
are kind and obedient.

Presence of risk 4.17 3 It is a health threat because not going to

factor of specific
diseases as a health

the health center for check-up immediately
can get worse situation that can be deficit.
It can be modified easily because the
person will be required medications to be
fully recovered from the disability. It can
have high preventive potential if the
family decides to go the health center
immediately for check-up of minor
disability that can probably be deficit. The
family did not totally perceive this as a
problem because they are not going to the
health center or hospital if the problem is
not yet serious and worse.

Polluted water 3.67 4 It is a health threat because we are not sure

supply as a health if the water that came from the artesian
aquifer is clean. Roncesvalles family used
threat
it as a drinking water for almost a decade.
This is easily modifiable if the family has
the knowledge about the total information
of the artesian aquifer. It is highly
preventive if health teaching about
artesian aquifer is performed. They did not
perceive this as a problem because they
are not yet having a problem while doing
this in almost a decade.

Lack of exercise 3.67 4 It is a health threat because lack of

activity as a health exercise has a cause of chronic diseases
and more. It is easily modifiable if the
threat
family will emphasize this and considering
as a problem in their condition. It is highly
preventive if health teaching about
advantages and disadvantages of exercise
is performed. The family did not consider
this as a problem because according to
them, their occupation and house works
are called exercise for them.
Unawareness of risk 3.67 4 This is a health threat because being
factor of specific unaware of the problem can get worse
situations for the health of the family. It is
disease as a health
easily modifiable if the family will be
threat aware and emphasize the problem even
though it just a minor problem. It is highly
preventive if proper nursing interventions
and health teaching are performed for the
betterment of the family. They do not
consider this as a problem hence they are
unaware in this kind of situation

knowledge on the the health center very seldom for their
health. It is partially modifiable because
resources for health
when crisis arise, the Roncesvalles family
care as a health tends to seek help from their relatives that
threat can provide them money if necessary. It
has a low preventive potential because
they still have insufficient income even
though 4 of the family members are
working to help each other. They are still
worrying somehow about the health threat
hence it is considered as a problem
needing urgent attention

Family size beyond 2.5 6 It is a health threat because it can affect

what family the health of the family especially health
care is not included in their priority. It is

partially modifiable because when crisis
adequately provide arise, the Roncesvalles family tends to
as a health threat seek help from their relatives that can
provide them money if necessary. It has a

though there is an intervention that may
address the problem, their family size
won’t change as well as their family
income even though 3 of their children are
working. The family seems to be worried
about the health threat they have hence it
