1. That I Have applied/ Guarantor for a loan facility of BDT 08.50 Lac in Favour of
“Shahjahain Enterprise” from The City Bank Limited Under “City Unsecured” Loan.
2. That due to inadvertent mistake/unawareness of my and/or concerned
authorities, My Name wrongly appears in the following documents:

person in Clause 2 & Clause 3 are same with same legal entity & responsibility, though
the mismatch and spelling mistake/error of name founded in the aforesaid documents
issued by different authorities.
And in relation herewith a certificate from local Chairman/Commissioner or any
other authentic supporting documents enclosed herewith as Annexure-A.
5. That I affirm and acknowledge that I shall be fully liable if any disputes arise out
of the above mentioned issue and I hereby further undertake that I shall indemnify and
keep BRAC Bank Limited fully harmless from each and every loss and damages it
would suffer due to such mismatch/error.
6. I do hereby declare that the contents of this affidavit are true to the best of my
knowledge and no part of it is false.
