Philippines, after having been sworn in accordance with law, do hereby depose and
state that:

I had been employed by NAME OF CORPORATION with principal office at

Bacolod City, Philippines, and I have voluntarily retired and terminated my
employment.

WHEREAS during the period of my employment and in consideration thereof, I

was fully paid and received from NAME OF CORPORATION in full and to my
complete satisfaction, all the salaries, wages, allowances, privileges, benefits and
other remunerations due me or I am entitled to, either by law or contract.

NOW THEREFORE, I hereby totally release and dischargeNAME OF

CORPORATION and all its officers, stockholders and employees from any debt,
damage or other form of payment and all liabilities arising from criminal, civil and
administrative actions and further waive and quit myself from claiming any further
benefits and or payments on account of future judgment or grant by law in
connection with or by reason of my previous employment with NAME OF
CORPORATION

This Deed of Release and Quitclaim was explained and read to me in the dialect
which I fully understand.

City, Philippines, and I have voluntarily retired and terminated my 28 years of
employment with the said employer since the year 2011-2019 .

WHEREAS during the period of my employment and in consideration thereof, I

was fully paid and received from Fernando Gonzaga in full and to my complete
satisfaction, all the salaries, wages, allowances, privileges, benefits and other
remunerations due me or I am entitled to, either by law or contract.

NOW THEREFORE, I hereby totally release and discharge Fernando Gonzaga

from any debt, damage or other form of payment and all liabilities arising from
criminal, civil and administrative actions and further waive and quit myself from
claiming any further benefits and or payments on account of future judgment or
grant by law in connection with or by reason of my previous employment with
Fernando Gonzaga.
